Objective: With the progress of science and technology, surgical instruments and surgical methods of continuous revolution and innovation, making minimally invasive surgery made great progress.Minimally invasive technology come into each of the surgical field, laparoscopic hepatectomy is also booming up.With the laparoscopic liver resection of the research report is increasing, both at home and abroad have a lot of laparoscopic hepatectomy on the safety and effectiveness of the literature. This paper retrospectively analyzed the patient's laparoscopic and open hepatectomy surgery in patients with information to explore the laparoscopic hepatectomy in the surgical treatment of liver disease in the clinical value. Methods: Retrospective analysis of April 2015 to November 2016 period,in line with the inclusion criteria in the Qingdao University Hospital Hepatobiliary and pancreatic surgery hospital,83 patients with laparoscopic hepatectomy(including liver tumor patients in 61 cases, hepatolithiasis patients 22 patients)and 82 patients with hepatectomy(45 patients with hepatocellular carcinoma and 37 patients with hepatolithiasis),compared with laparoscopic hepatectomy and laparoscopic hepatectomy for the treatment of liver disease.According to their surgical methods were divided into LH(Laparoscopic hepatectomy)group and OH(Open hepatectomy)group. The operative time, intraoperative blood loss, intraoperative blood transfusion ratio, the proportion of patients with intrahepatic obstruction, intraoperative hospital stay, postoperative liver function recovery and postoperative complications were compared between liver tumor patients and hepatolithiasis. To compare the short-term clinical evaluation of laparoscopic hepatectomy and laparoscopic hepatectomy to evaluate the short-term efficacy and safety of laparoscopic surgery in the treatment of liver tumors and hepatolithiasis. All data were analyzed by SPSS 19.0 software,P<0.05 showed significant difference. Results: All patients were successfully completed,no cases of perioperative death,by analyzing the laparoscopic hepatectomy patients(LH group)and open hepatectomy group(OH group)statistics, found that both liver cancer patients or hepatobiliary, there were no significant differences in the proportion of sex, age, lesion location, lesion size, percentage of ChildA patients and preoperative liver function(P> 0.05).However,there was no significant difference between the LH group and the OH group in the operation time, the proportion of blood transfusion in patients and the proportion of hepatic portal obstruction during operation(P> 0.05).In the intraoperative blood loss, postoperative hospital stay, postoperative liver function recovery rate were better than OH group, the difference was statistically significant(P< 0.05).Conclusion: Compared with laparotomy, laparoscopic hepatectomy has the advantages of mild trauma, rapid postoperative recovery, shorter hospital stay and so on.It is safe and feasible.
