The Clinical Features Analysis Of Multiple Primary Cancer Associated With Oral Cancer

Objective: Objective to analyze the clinical features of 40 cases of multiple primary cancers associated with oral cancer in oral cancer of 1679 cases.Discuss the related factors of clinical diagnosis and incidence of oral cancer associated with MPC,to raise awareness of clinicians on oral cancer related MPC strengthen follow-up,prevent the misdiagnosis of MPC.Finally,it can achieve the purpose of early diagnosis,early treatment and improvement of prognosis of oral cancer associated MPC.Methods:From January 2010 to December 2016 were collected in the Fourth Affiliated Hospital of Hebei Medical University,Department of Stomatology hospitalized a total of 1679 cases of patients with oral cancer.According to the classic diagnostic criteria proposed by Warren and Gates,40 patients with oral cancer associated with MPC were selected.The first MPC part with oral MPC is 4 cases,and the second MPC part with oral MPC is 36 cases.The clinicopathological features of patients with MPC associated with oral cancer were analyzed retrospectively,including gender,age,smoking history,genetic history,pathological type,clinical stage.Statistical analysis was performed using SPSS21.0,and the chi square test was used to count the data.P<0.05 showed that the difference was statistically significant.Result:1 Male and female ratio of MPC in oral cancer related group 1.1:1.In the control group,the ratio of male to female was 1.5:1.The difference between the two groups was analyzed by chi square test(X~2=6.06,P=0.01<0.05),and the difference was statistically significant.MPC group of oral cancer diagnosed with an average age of 66.3 + 10 years old,the average age of diagnosis of solitary OC group 60.3 + 17.2 years,two groups of patients with oral cancer diagnosed age chi square test(X~2=6.06,P=0.01<0.05),the difference was statistically significant.Clinical OC patients in the MPC group stage distribution is uniform,which ? and ? in 21 cases,19 cases of stage ? and ?.Single phase OC clinical stage ?,? 22 cases,?,? phase of 18 cases,OC group and MPC group for clinical staging of the chi square test,X~2=0.05,P=0.823>0.05,no statistical difference.MPC group of 17 patients with a history of smoking in male patients were male,accounting for 80.9%;6 cases with family history(15%).Single OC control group(13 cases)had a history of smoking,all of which were male,accounting for more than 54.1% in male patients,and none of the patients had a family history in 40 cases.2 In the MPC group,there were 39 patients with double MPC,1 patients with three MPC.There were 10 cases of synchronous multiple primary carcinoma and 30 cases of metachronous multiple primary carcinoma.There were 23 patients found the second primary cancer after diagnosing the first primary cancer 5 years.There were 17 cases with an interval of more than 5 years,and the oldest was about 20 years.3 Other parts of oral cancer group MPC in malignant tumor in esophageal cancer(30%)was the most common,followed by lung cancer(15%)and cardiac cancer(12.5%),breast cancer(10%),colon cancer(7.5%),thyroid carcinoma(5%).The pathological type of MPC in group OC was squamous cell carcinoma,and the most common site was gingival.4 Oral cancer related MPC group found in oral cancer before radiotherapy in 10 patients(25%).Oral cancer underwent surgical treatment in 23 cases(57.5%)in MPC group;Oral cancer underwent surgical treatment in 34 cases(85%)in OC group.Two groups of surgical operation rate of chi square test,X~2=7.384,P=0.007<0.05,the difference was statistically significant.Conclusion:1 Multiple Primary Cancer Associated with of Oral cancer is related to age,sex,heredity and smoking,not related to clinical stages.2 The MPC associated with oral cancer is more common in double cancers,and the three MPC are rare.Most of the primary sites associated with oral cancer are common in the esophagus,lung,stomach,breast,and so on.3 Squamous cell carcinoma was the main pathological type of MPC in oral cancer.4 The MPC associated with oral cancer may be associated with radiotherapy for the first primary cancer.In our hospital,the MPC associated with oral cancer was lower than that of single oral cancer in second primary cancers.The resection rate of second primary cancers in patients with synchronous multiple primary carcinoma was lower than that of metachronous multiple primary cancers.
