| Objective: Lumbar disc herniation(LDH)is a clinical common disease in department of orthopaedics,frequently-occurring in young adults.It has the characteristic such as touching painless,repeated attacks and causes patients great burden and pain.Lumbar disc herniation refers to the syndrome of a series of symptoms such as waist and leg pain and numbness caused by fibrousring rupture and pulpiform nucleus protrusion,oppression and stimulate the surrounding tissue due to various reasons.The disease has not a definite conclusion in clinical treatment and everyone has their own viewpoint.But it has only two treatments: surgical and conservative therapy.Excision of herniated disk,vertebral fusion,internal fixation,and so on,are common surgical treatments.But it arises complications such as recurrence and vertebral unbalance if excision of herniated disk.After vertebral fusion,we can discover adjacent segmental lesions,lower back stiffness.And internal fixation has metal foreign body reaction and it is a high cost.All in all,surgical treatment havs disadvantages of big traumas,high risk and high cost,so patients are often not easy to accept.But in conservative therapy,traditional Chinese medicine has great advantages and characteristics and is being constantly research and explore featured with merits of convenience,simply,cheapness and good effect.This study focuses on hospitalized patients suffering from disease of qi stagnation and blood stasis type of lumbar disc herniation in orthopaedics department of our hospital.Study the clinical curative effects by using the treatment approach of Zhanjin Huoxue Prescription matching with Lumbar bonesetting manipulation and provide an effective treatment for clinical.Methods: 72 cases of hospitalized patients(suffering from qi stagnationand blood stasis type of lumbar disc herniation)fall events occurred in Orthopedics three affiliated to Hebei Traditional Chinese Medicine were choosed from April 2015 to April 2016.It was randomly divided into treatment group and control group(each group has 36 cases).In the treatment group,patients oraled Zhanjin Huoxue Prescription(consisting of Angelica 12 g,Ligusticum wallichii 10 g,Radix Achyranthis Bidentatae 9g,Fructus Aurantii15 g,turmeric 15 g,Root of Doubleteeth Pubescent Angelica 15 g,Dragon's Blood 6g,Atractylodes macrocephala 20 g,Poria cocos15 g,Angelica dahurica15 g,musk 0.1g,Peach kernel 10 g,Safflower 10 g,Eucommia 15g),and the herbs were provided from pharmacy affiliated to Hebei Traditional Chinese Medicine,decocted by boiling room use soft fire,extract 2 times and take juice 450 ml,then take three times a day,one dose a day patients should not eat the cold food and greasy food.During the period of the medicine,we cooperated with lumbar bonesetting manipulation,and it was applied every three days for a 15-day period.In control group,patients were treated only by lumbar bonesetting manipulation.Both groups began the back muscle function exercise in 1 week after treatment.Patients adopted five supporting method,3times a day,5 sets each time with 3 minutes rest every set,and paused for5-10 seconds between each movement.In the stage of therapy,other any medicines of Chinese and western or Lumbar bonesetting manipulation could not be applied.This research compared the basic information such as gender,age,course of the disease by SPSS21.0 statistic software.After treatment,studied the changes of Visual analogue score and the lumbar vertebra JOA score before and after treatment,then evaluated curative effect with reference to the assessment criteria of lumbar intervertebral disc from Guideliness for the Clinical Researchof Chinese Medicine NewDrugs.Results :1 There was no statistically significant difference between treatment group and control group,and they were comparable,P>0.05.2 The difference was not statistically significant between treatment group and control group before treatment in Visual analogue score(P>0.05);aftertreatment,the two group's scores were both decreasing.And treatment group is better than that of control group,difference has statistical significance(P<0.01).3 The treatment group and control group in lumbar JOA score before treatment,there was no statistically significant difference(P>0.05);In the two groups after treatment lumbar JOA scores were increased,and the treatment group is better than that of control group,difference is statistically significant(P<0.01).4 Curative effect evaluation: in the control group of 36 cases of patients after a course of treatment,10 cases were cured,11 cases effectively,improvement in 11 cases,4 cases were not cured,the total effective rate was88.9%;And in treatment group(36 patients after a course of treatment),16 cases were cured,14 cases were effective,4 cases were improved,2 cases were not.the total effective rate was 94.4%;The efficient comparison of similar between the two groups have statistical significance(P<0.05).Conclusions:Zhanjin Huoxue Prescription play effects on Zhanjin Huoxue?promoting qi circulation and relieving pain ? Qiangyao Zhuangshen,and Lumbar bonesetting manipulation can adjust spine biomechanics angle,ease the pressure on the intervertebral disc and spinal canal.By this way,It is able to make the shape and size of intervertebral foramen to change,remove or alleviate nerve root compression and let the organization has resumed to normal coordinate motion state.In this research,we can give full play to the characteristics of traditional Chinese medicine and dialectical application,by cooperating formulas with methods.So it can effectively alleviate the symptoms of patients before treatment and improve the quality of life for patients.Zhanjin Huoxue Prescription matching with Lumbar bonesetting manipulation have a pretty good clinical therapeutic effect on qi stagnation and blood stasis type of lumbar disc herniation.And this therapeutic method is safe,convenient and low cost,easy to learn.So it is worthy of further clinical research and extension. |
