Objective: The elderly cardiovascular medical patients by mini nutritional assessment(MNA)and patients with nutritional risk screening scale(NRS2002)to assess the nutritional status, factors on the effects of malnutrition, and observe its effects on clinical prognosis, understand the current situation of nutritional support in hospitalized elderly cardiovascular patients. Methods: In this study, second patients admitted to the Department of cardiovascular medicine of Hebei Medical University from January 2016 to December 2016 were selected by systematic sampling method. The patients were enrolled in the hospital for 72 hours to complete the nrs2002 nutritional risk screening,Mini Nutritional Assessment Scale(MNA)questionnaire.Patients were collected for nutritional support and nutritional support during hospitalization. Before the patient was discharged, the patients were given the nrs2002 nutritional risk screening and the mini nutritional assessment scale questionnaire to investigate the nutritional status of the patients before discharge. The statistical analysis of this study were analyzed by SPSS 13 statistical software. The results of the statistical description of the measurement data were expressed as "x±s".The measurement data using rate, frequency or constituent ratio.Nutrition and normal nutrition group and abnormal group, compared with the existing nutritional risk and nutritional risk does not exist between the related factors analysis of variance, non parametric test X 2 test or rank transformation. Two kinds of scales were used to evaluate the nutritional risk and the results of the laboratory standards were compared using the consistency test. The a=0.05 was the test level, and the difference of P<0.05 was statistically significant.Results:1 Two kinds of assessment method to evaluate the nutritional risk and malnutrition results compared to laboratory. All patients were MNA,NRS2002 and laboratory examination. The results showed that he detection rate was 48.7% of laboratory examination scoring method of malnutrition was positive, the application of MNA scale method for nutritional abnormalities in patients the detection rate was 44.3%,there was significant difference compared with(X2=4.846,P=0.028).The detection rate was 34% of nutritional risk in patients with NRS2002 scale score of,which was significantly different from that of laboratory examination(X2=27.546,P<0.001).2 The comparison of two kinds of scales and laboratory indicators to check the consistency of malnutrition. The rate of malnutrition risk was 34%in the NRS 2002 scale, and the concordance rate with the laboratory index was,and the kappa value was 0.25,and the NRS 2002 scale was inconsistent. The incidence of malnutrition was 44.3% by MNA scale, and the concordance rate of malnutrition with the laboratory index was 81.2%,kappa was 0.62,The MNA scale and laboratory indexes were highly consistent with the presence of nutritional status.3 The nutritional status of different age groups, gender and BMI were compared. The nutritional status of elderly patients with cardiovascular diseases were evaluated by MNA scale.Comparison of age,the age group of60-69 patients, the age group of 70-79 patients, aged 80 groups of patients with nutritional abnormalities accounting for 27.5%,35.6% and 56.6% of the patients, older patients significantly higher than the incidence of abnormal nutrition the younger(P<0.001).Sex between male and female patients with nutritional abnormal rate were 43.6% and 46.2%,there were no significant difference between male and female patients(P=0.737).According to BMI group, lean group(<18.5 kg/m2),normal group(18.5-23.9 kg/m2)and obese patients(>24 kg/m2)with abnormal nutrients, accounting for 100%,47% and30.6% of the patients, the smaller the BMI patients with abnormal nutrition were significantly higher(P <0.001).4 The influence of lifestyle and disease history on nutritional status was evaluated by MNA scale score for the elderly patients with cardiovascular diseases.Smoking and non-smoking group nutrition in patients with abnormal rate were 46.9% and 42.4%,no significant differences between the two groups(P=0.337).Drinking and non drinking group nutrition in patients with abnormal rate were 65.8% and 40.1%,significantly increased the incidence of abnormal drinking nutrition(P<0.001).The incidence of abnormal nutrition was 60%,and the incidence of malnutrition in patients with no history of disease was 38.3%.There was a significant increase in the incidence of disease in individuals with a history of disease(P <0.001).5 Comparison of different nutritional status of patients and measured laboratory index index. The scores of the nutritional status of elderly patients with cardiovascular disease were evaluated using MNA,comparison of different nutritional status of patients and measured laboratory index index.Three groups of patients with albumin and hemoglobin levels were significantly different(P<0.05)three groups of patients with mid arm circumference, there exist obvious differences in non dominant hand grip and Maximum leg circumference(P<0.05).There is no significant difference between the triceps skinfold thickness in patients of the three groups(P=0.068),the three groups of patients with upper arm muscle circumference between no significant difference(P=0.073).6 Analysis of nutritional support in patients with different nutritional status, and to evaluate the nutritional status of elderly patients with cardiovascular diseases by MNA scale. In the normal group, there were 2cases of nutritional support, all of which were enteral nutrition support. There were 15 patients of nutritional support in the patients with latent malnutrition; There were 31 patients nutritional support in the malnutrition group. There were significant differences in nutritional support between the three groups(X2=105.9,P<0.001).7 The nutritional status of the patients at the time of discharge and admission was compared. The nutritional status of the elderly patients withcardiovascular diseases were evaluated by MNA scale. At the time of admission,270 cases of patients with normal nutrition group, nutrition group with abnormal 215 cases;patients were discharged,284 cases of patients with normal nutrition group, nutrition group with abnormal 201 cases, patients with no significant difference between admission nutritional status(X2=0.921,P=0.631).Conclusions:1 MNA scale and laboratory indicators to determine the incidence of malnutrition is similar, and the consistency is good, the elderly patients with cardiovascular disease in patients with malnutrition detection effect is better than the NRS2002 scale.2 The incidence of nutritional abnormalities in elderly hospitalized patients with cardiovascular disease was influenced by age,BMI,drinking history, and the history of individual diseases.3 Cardiovascular medical patients have high incidence of malnutrition, malnutrition and nutritional support in patients with low rate.
