Background:For the past few years,the nutritional status of hospitalized patients has been payed more and more attention by clinical research institutes in China or other countries. The correlated researches showed that the patients in malnutrition status often had higher morbidity and mortality.But there are too many difficults in assessment of nutritional status of hospitalized patients.otherwise,there has been no unified diagnostic criteria in the world by now. There are many nutritional screening or assessment tools being used in clinical institutions, such as subjective global assessment(SGA),Mini Nutritional Assessment (MNA).but recently,series of studies showed that these two tools had deficiency of applicability,effectivity and reliability.In 2002, European Society of Parenteral and Enteral Nutrition (ESPEN) introduced a new nutritional screening tools NRS2002.This is a new methods which can assess the patients' nutritional status easily and reliably.It has been widely used in many European countries.But it had not been used widely in China. Objective:To assess the nutritional status of hospitalized patients in China by NRS2002. To investigate the incidence rate of patients with malnutrion,the incidence rate of patientswith nutritional risk. the incidence rate of patients received nutritional supports,and make statistical analysis.And to evaluate the feasibility of NRS2002 in China. Methods:The study population consisted of new patients admitted to 6 departments in Sir Run Run Shaw Hospital, include of neurology department, pulmonary medicine department, gastroenterology department, nephrology department, general surgery department and thoracic surgery department.between August 1, 2006 and February 28,2007. Assess the nutritional status of all the patients by NRS2002.And record the general datas,the body mass index (BMI) and the NRS scores of all patients. The BMI was adopted by Chen Chunming standard which lowered than 18.5kg/m~2 as malnutrition.The patients who could not use BMI would be examined the serum albumin (sALB) which lowered than 35g/L as malnutrition. Results:A total of 538 patients were admitted to this study,included 302 male patients(56.1%) and 236 female patients(43.9%),the average age is 53.9.The research for the applicable rate of BMI of the patients showed that 93.3% patients of nephrology department could use BMI,which was the highest.the next is general surgery department(85.1%) and thoracic surgery department(81.4%).Patients in the other three departments had lower BMI applicable rates.The research for the NRS scores of the patients showed that the highest 44.5% patients in pulmonary medicine department had the risk of malnutrition(NRS score≥3), the next is gastroenterology department(39.4%),and neurology department(28.4%).The patients in those two surgical deparments had relatively lower rate of nutritional risk.In general surgery deparment 80% patients' NRS scores were 0 or 1,and in thoracic surgery department this rate was also approach 70%.The research for the BMI of the patients showed that the highest 28.2% patients in gastroenterology department were in malnutrition status(BMI<18.5kg/m~2),the next is pulmonary medicine department(15.7%).And patients in general surgery departmentand neurology department had the highest rate of overnutrition.In General Surgery department 38.1% patients'BMI was over than 24kg/m~2,and in neurology department it was 35.6%.The research for the nutritional support of the patients in nutritional risk showed that all the departments had not provided the sufficient nutritional supports to the patients which needed nutritional supports.The rate of nutritional support in these departments was 30-50%. Conclusions:This study showed that malnutrition is a common problem in hospitalized patients in China,but we had not payed enough attention to this problem. NRS2002 is a kind of new nutritional screening tool which can assess the patients' nutritional status easily and reliably. So it is worth to spread NRS2002 in China.
