A Clinical Study On The Treatment Of “Jin Gou Diao Yu” Acupuncture For Spastic Paralysis After Ischemic Stroke

Objective: To observe the clinical effect of “Jin gou diao yu” acupuncture for spastic paralysis after ischemic stroke and provide clinical evidence for acupuncture treatment of this disease.Methods: According to clinic spasticity index,all of 66 patients were allocated to three layers: slight,moderate and severe,and divided into the trial group and the control group adopting layering randomized method,each group of 33 cases.The basic treatment for the two groups were given routine neurological medication.The trial group accepted “Jin gou diao yu” acupuncture,and selected the acupoints of Tianfu(LU 3),Chize(LU 5),Neiguan(PC 6),Daling(PC 7),Liangqiu(ST 34),Yinlingquan(SP 9),Sanyinjiao(SP 6),and Zhaohai(KI 6),needling each time per day,retaining for 30 minutes each time.The control group accepted Bobath therapy,each time per day,45 minutes each time.The treatment for the two groups were five days of one course,two days for rest,and four period of treatment in total.The modified Ashworth Rating Scale(MAS),clinical spasticity index(CSI),Fugl-Meyer Assessment(FMA)and Barthel index(BI)were used to evaluate the effect of two groups of patients at three time points: before treatment,treatment for 14 days,and treatment for 28 days.Results: 1 Clinical effect of the two groups of patients before and after treatmentAfter 14 days of treatment,the total effective rate on upper limbs was 77.4% in the trial group and 66.7% in the control group,and was no statistically different(P>0.05),and the total effective rate on lower limbs was higher in the trial group(83.9%)than control group(70.0%)(P<0.05).After 28 days of treatment,the total effective rate on upper limbs was higher in the trial group(87.1%)than control group(73.3%)(P<0.05),and the total effective rate on lower limbs was higher in the trial group(93.5%)than control group(76.7%)(P<0.05).2 CSI score of the two groups before and after treatmentAfter treatment for 14 days and 28 days the CSI score of the two groups was lower than before treatment(both P<0.05).And after treatment for 14 days and 28 days,the CSI score in the trial group was lower than the control group(both P<0.05).3 FMA score of the two groups before and after treatmentAfter treatment for 14 days and 28 days,the FMA score of the two groups was higher than before treatment(both P<0.05).And after treatment for 14 days and 28 days,the FMA score in the trial group was lower than the control group(both P<0.05).4 BI score of the two groups before and after treatmentAfter treatment for 14 days and 28 days,the BI score of the two groups was higher than before treatment(both P<0.05).And after treatment for 14 days and 28 days,the BI score in the trial group was lower than the control group(both P<0.05).Conclusions: “Jin gou diao yu” acupuncture has effect on rehabilitation of spastic paralysis after ischemic stroke,and can effectively reduce the patients with limb muscle tension,relieve limb spasticity,improve the motor function of the limbs and improve the patients' activities of daily living and quality of life.Furthermore,the effect is better than Bobath therapy.
