Comparative Efficacy Of PFNA And Inverted LISS Plate For The Treatment Of A2 Unstable Type Of Intertrochanteric Fractures

Objective To explore the clinical effect of PFNA and inverted LISS plate for the treatment of A2 unstable type of intertrochanteric fractures,which provides the theoretical basis to better guide clinical work.Methods Retrospective analysis during August 2008 to February 2016 in our department to accept PFNA and inverted LISS plate treatment with a follow-up type 40 cases of A2 unstable type of intertrochanteric fractures in patients with data.37 cases with entire follow-up,3 cases were lost.There were 19 males and 18 females,at age rang of 48-69 years,average age(58.6±10.4).18 cases using PFNA fixed,19 cases with inverted LISS plate fixed,compared two groups of patients with incision length,operation time,surgical blood loss and fractures of the clinical healing time,postoperative function Harris hip score,and surgical complications.All the data have been gotten was analyzed by SPSS 17.0solfware.Results 37 cases with the entire follow-up from 12 to 18 months(mean 14 months).PFNA and inverted LISS plate group surgery time,respectively(62.4±10.3),(103.5±16.7)min,surgical blood loss,respectively(175.4±31.6),(283.7±27.4)ml,postoperative incision drainage,respectively(69.3±6.5),(125.4±5.8)ml,incision length,respectively(6.2±1.3),(7.2±1.9)cm,hospitalization time,respectively(7.1±1.2),(7.3±1.1)day,postoperative weight time,respectively(17.8±1.6),(38.0±2.4)day,bone fracture clinical healing time(12.2±1.9),respectively(11.1±2.3)for weeks.The incision length,operation time,blood loss and drainage,postoperative function Harris hip scor,weight loading time of the inverted LISS plate group higher than the PFNA group,the difference was statistically significant(P <0.05).The fractures of the clinical healing time had no statistical significance(P>0.05)between the two groups.Conclusion PFNA and inverted LISS plate in the treatment of intertrochanteric fractures can obtain a satisfactory effect.PFNA is superior to inverted LISS plate is the treatment of A2 unstable type of intertrochanteric fracture.PFNA is much more suitable for patients who have many advantages,short intraoperative time,small trauma,less bleedling,less complication,simplicity of operator,and significant clinical efficacy,which is worth spreeding.
