Objective:Based on the continuous investigation,the data of patients with headache in Qinghai province native outpatient clinic were analyzed.To understand the past diagnosis and treatment of patients,analyze the causes in the neurology clinic and the relationship between anxiety and depression disorders.The purpose of this study is to provide clinicians with the diagnosis and treatment of headache associated with anxiety and depression disorders.Methods:Through a self-designed headache questionnaire,we had collected to headache patients with complaints(in line with the international standard of ICHD-II diagnosis and treatment)in Affiliated Hospital of Qinghai University from December 2015 to December 2016.According to the diagnostic criteria,300 patients with headache in Qinghai province were divided into three groups:the migraine group(n = 131),the tension headache group(n = 128),and the other types of headache group(n = 41).To investigate the etiology,related factors,predisposing factors,co-morbidities and so on,SPSS21.0 statistical software was used for analysis to compare these three groups.Results:300 cases of headache patients,the proportion of primary headache was 87.67%,the proportion of secondary headache was more than 6.33%,neuralgia accounted for 3.33%,unexplained headache accounted for 3.33%.Primary headache migraine in 131 cases,accounted for 43.7%,tension headache in 128 cases,accounting for 42.7%,4 cases of cluster headache and other trigeminal autonomic nervous headache,accounting for 1.7%;The female patients have 206 cases in total of 300 cases,accounting for 68.7%,mainly in the age of 30 to 49 years old,of which the incidence of female patients gradually increased in the age of about 30.At the age of 40 to reach the peak.The highest level of education in junior high school,accounting for 30.7%.Occupation of farmers and herdsmen,accounting for 41%.With a family history of headache ratio reached 26.7%.In 300 patients,there were 122 patients with anxiety and depression,accounting for 40.7%;Among them,the patients with tension headache were more likely to have anxiety disorders,Comparing with the three groups,the difference was statistically significant(P < 0.05).Among all the patients with headache,39.3% patients with sleep disorders,Higher proportion of patients with migraine complicated with sleep disorders.The difference between the three groups was statistically significant(P < 0.05).In 122 patients with anxiety and depression,there were 47 patients with migraine,the patients with anxiety and depression were more likely to have migraine.The difference between the three groups was statistically significant(P < 0.05).In patients with anxiety and depression,59 patients with tension headache.Among them,the proportion of patients with tension headache was higher in anxiety group.There was a significant difference in depression group,anxiety group and anxiety depression group(P < 0.05).259 cases of migraine and tension headache in patients with migraine patients are vulnerable to get in a draught,sunlight,noise,alcohol,weather changes induced.And tension headaches are more susceptible by emotional changes,fatigue,the difference was statistically significant(P < 0.05).All patients with headache,The headache frequency(i.e.greater than or equal to 15 days / month)in patients with anxiety and depression were higher proportion.Compared with non-comorbidity group,the difference was statistically significant(P < 0.05).Conclusion:In Qinghai Province,headache patients are more common in primary headache,migraine and tension headache patients mainly.Tension headache,migraine patients are more common in young women and farmers and herdsmen.Patients with lower education level,the proportion of family history of headache was higher in migraine patients.Patients with tension headache were more likely to have comorbid anxiety disorder.Migraine is more likely to be associated with sleep disorders,the interaction between primary headache and anxiety and depression,the frequency of headache attack is closely related to anxiety and depression.
