A Preliminary Study On The Characteristics Of Chronic Primary Headache State Of Mind (anxiety And Depression) And TCM Syndromes

Objective:To analyze the mood state (anxiety and depression disorders) with the TCM syndrome of chronic primary headache and find their character in distributions. To provide an objective basis for the treatment of chronic primary headache.Methods:Collecting 51 cases of outpatient and inpatient randomly in chronic patients with primary headache in Beijing university of Chinese medicine east hospital. Standard of syndromes in TCM according to the standard syndromes of headache diagnosis and effective evaluation standard made by 1997,encephalopathy emergency group in the state administration of TCM. Headache diagnosis standard according to the 2004 "2th edition.headache diagnosis standard made by the international headache society". And anxiety and depression disorders according to 10th edition,ICD-10. Collecting the four diagnostic information and basic data by survey scales. Building a database by EpiData3.1,all data is using SPSS20.0 statistical software for statistical analysis.Results:In 51 cases of chronic primary headache, the minimum age is 22 years old and the maximum is 72. The shortest duration is 2 years and the longest is 42.The percent of women patients is 74.5%,while men is 25.5%. The percent that have family history in headache is 43.1%.The overall trend in types of chronic primary headache is same with the trend of headache whose cultural degree lower than middle school:Migraine >TH>Migraine combines with TH. Past treatment plan is different from each others,western medicine account for 41.2%,TCM account for 41.2%,western medicine combine with TCM is 13.7%. The overall trend of VAS score in general and severe headache are the same:Migraine combines with TH>Migraine>TH.In 51 cases of chronic primary headache,the percentage without anxiety and depression disorders is 47.0% and with it is 53.0%. Women with anxiety and depression account for 41.2% and men is 11.8%. Pureanxiety account for 17.7%, Pure depression account for 11.8%, anxiety combine with depression account for 23.5%. The minimum age in the group of anxiety combine with depression is 37 years old and maximum is 62. The shortest duration in the group of anxiety combine with depression is 4 years and the longest is 37. The group of anxiety combine with depression’s HIT-6 score is highest,and followed by pure anxiety, pure depression and then without anxiety and depression disorders.The overall trend of VAS score in general headache is:pure anxiety>anxiety combine with depression>without anxiety and depression disorders> pure depression.In 51 cases of chronic primary headache,Migraine combines with TH has significantly positive relation with VAS score in general headache. TH has significantly negative relation with VAS score in severe headache. Without anxiety and depression disorders has significantly negative relation with HIT-6 score in headache. Anxiety and depression has significantly positive relation with HIT-6 score in headache. Pure anxiety has significantly positive relation with VAS score in general headache. Migraine combines with TH has significantly negative relation with without anxiety and depression disorders,while significantly positive relation with pure anxiety. Migraine has positive relation with wind fire, wind phlegm, qi deficiency and wet resistance,while significantly negative relation with qi and blood deficiency. TH has significantly positive relation with qi and blood deficiency. Migraine combines with TH has significantly positive relation with wind and blood stasis.Qi deficiency and wet resistance has significantly negative relation with without anxiety and depression disorders,and significantly positive relation with anxiety and depression disorders. Pure depression has significantly positive relation with wind fire.Conclusion:1 Family history of headache play an important role in the etiology of primary headaches.2 Low degree of culture is a factor that courses chronic primary headache with long duration.3 Chinese medicine,or combining with western medicine treatment can broaden the field in primary headache.4 Migraine combines with TH may aggravate the degree of headache and have close relation with anxiety.5 With anxiety may aggravate the degree of general headache. With anxiety and depression disorders may produce a greater impact on patients.6 TH is common in qi and blood deficiency,while Migraine combines with TH is wind and blood stasis.7 Pure depression is common in wind fire,while anxiety and depression disorders is common in qi deficiency and wet resistance.
