A Study On Construction Evaluation Indicator System Of Nursing Competency For Gerontological Nurses

Objective:By analysis the current situation of gerontological nursing and care needs for the elderly inpatient,combined with the training status of gerontological nurses in foreign countries.This study aimed to construct evaluation indicator system of nursing competency for gerontological nurses(GNs)in China,and then to rank the importance among those indicators.It can provide references for the training and evaluation of GNs as well as the talent system development of gerontological nursing.Methods:1 A comprehensive research of the literature was performed to locate articles about Nursing competence for GNs,The following online databases were utilized in this search:Cumulative Index of Nursing and Allied Health Litera-ture(CINHAL),Pub Med,MEDLINE,CNKI and FMRS et al.The following search terms were used alone and in combination: gerontological nursing,nursing competence,nurse competence,competence,gerontological nurse,geriatric nurse,old people,Relevant studies were retrieved according to the references for more details.Literatures were used to understand the status quo of nursing competencies of GNs abroad,then analysis the location of GNs in china conditions.after repeated discussion of research group,the draft of the GNs’ nursing competencies framework were formulated.2 twenty gerontological nursing experts in Taiwan,Beijing,and Hebei were consulted by a two-round Delphi survey.According to expert opinion and inquiry index analysis,we screened,modified and revised the index system,then we determined the GNs’ competence index system ultimately.The representativeness and credibility of the Delphi method were tested by the positive coefficient,authoritative coefficient,coordination coefficient,the concentration of expert advice and the positive coefficient of experts.Results:1 The nursing competence evaluation index system of the GNs’ was constructed,including 6 dimension level indicators,15 second level indicators and 54 third content indicators.The weight score of 6 dimension indicators were as follows:nursing concept(0.1729),critical thinking(0.1636),nursing practice(0.1786),communication and cooperation(0.1711),education and training(0.1596),professional development(0.1543).2 Scientific analysis of index system of nursing competence for GNs:After the first-round Delphi survey was consulted,according to expert opinion,combined with the index selection standard that the importance assignment was above 3.5,the coefficient of variation(CV)was less than 0.25,we modified 5 content indexes and increased 3 content indexes by discussion repeatedly.After the Second round,according the index consistent standard that the importance assignment was above 4.0 and CV was less than 0.25,expert opinion were presented in concentration degree analysis and all level index important assignment was above 4.11;the content validity of I-CVI were above 0.78,the coefficient of variation was below 0.25.3 Reliability analysis of index system of nursing competence for GNs:Expert positive coefficient of two rounds were 95% and 100%respectively.15 experts(75%)proposed amendments for the first round and 3experts(15%)proposed amendments for the second round.The expert authority coefficient of Delphi was 0.825 and the coordination coefficient was between 0.196-0.263.(P<0.01).Conclusions:Based on literature analysis,the study constructed the evaluation indicator system for GNs by Delphi Panel methodology that include 6dimension level indicators,15 second level indicators and 54 third content indicators.The weight score of 6 dimension indicators were as follows:nursing concept(0.1729),critical thinking(0.1636),nursing practice(0.1786),communication and cooperation(0.1711),education and training(0.1596),professional development(0.1543).The studies was concrete andthe content was detailed.We hope it can provide reference for the training and evaluatting of GNs.
