Objective: To compare the clinical effects of treatment for simple anal fistula by fistula elimination combined with aboral drainage skin-bridge sparing surgery vs.anal fistulectom,and to observe the postoperative conditions,so as to explore the advantages of fistula elimination combined with aboral drainage skin-bridge sparing surgery for the treatment of simple anal fistula.Methods: The research subjects were selected from those patients with anal fistula who had received surgery in The People's Hospital of Qian'an between May 2015 and May 2016 and the conditions met the research requirement.The patients were randomly assigned into a treatment group and a control group,with 50 cases in each group.The patients in the treatment group were treated with fistula elimination combined with aboral drainage skin-bridge sparing surgery;the control group was given anal fistulectomy for treatment.A 6-month follow up was given to both groups.The average wound healing time,recovery rate,anal function and shape,and the recurrence rate of anal fistula during the follow up were observed to discuss the safety and effectiveness of the surgery plans employed in this research.Results:1 The length of hospital stay was(13.2±2.6)days in the treatment group,significantly shorter than that in the control group,which was(19.4±6.2)days.Statistical analysis exhibited the difference was statistically significant(P<0.05);2 In terms of the wound healing time,it was(16.2±2.6)days in the treatment group and(25.3±5.9)in the control group.Through statistical analysis,the comparison showed statistically significant difference between the two groups(P<0.05),which indicated that the surgery plans taken in the treatment group could help to shorten the wound healing time;3 The postoperative infection rate in the treatment group was 4%,lower than the 6% in the control group;however,the statistical analysis showed there was no statistically significant difference existed between the two groups(P>0.05).4 As regards with the postoperative pain indexes,the pain scores registered at the day and 14 days after the surgery were(4.4±1.1)and(0.0±0.0)in the treatment group,and(5.3±1.3)and(1.3±0.5)in the control group.The comparison between the two groups was statistically significant(P<0.05);However,the scores registered 3 days and 7 days after the surgery,which were(3.1±1.2)and(1.5±0.8)in the treatment group,and(3.3±0.8)and(1.9±1.4)in the control group,showed no statistically significant difference between the two groups(P>0.05);5 The comparison of catheterization rate,intramuscular injection of Prostigmine,physiotherapy and uroschesis showed no statistically significant difference between the two groups(P>0.05);6 The postoperative urination score at the day was(1.1±0.9)in the treatment group and(3.6±1.2)in the control group,with the difference between the two groups(P<0.05),which indicated that the combined surgery was with better urination effect compared with the simple surgery;7 The incidence of postoperative bleeding was 14% in the treatment group,lower than the 36% in the control group,with statistically significant difference between the two groups(P<0.05);8 X2 was employed to scale the rectal touch in the two groups,P=0.528,>0.05,without statistically significant difference between the two groups(P<0.05);9 Postoperative incontinence in the two groups was scaled by X2,P=0.823,>0.05,without statistically significant difference between the two groups(P<0.05);10 The effective rate of treatment was higher in the treatment group,which was 94%,and it was 78% in the control group,and through statistical analysis,the comparison was statistically significant(P<0.05).This showed that,for the treatment of simple anal fistula,fistula elimination combined with aboral drainage skin-bridge sparing surgery was more advantageous;11 There was no recurrence occurred in either group during the follow up(recurrence rate was 0% in both groups),without statistically significant difference between the two groups(P<0.05);Conclusions:Fistula elimination combined with aboral drainage skin-bridge sparing surgery is a new,safe and effective way in the treatment of simple anal fistula,which can cure the anal fistula while improve the life quality and satisfaction of the patients.Therefore,it is of significant value to be popularized in clinical application.
