Objectives Analyze the influencing factors of acute stress disorder in patients with acute myocardial infarction,and to provide the theoretical basis for the development of early nursing intervention.Methods 1 The patients with acute myocardial infarction as the object were selected from September 2015 to August 2016 in Tangshan Gongren Hospital department of cardiology in this study.The patients were randomly selected from the Stanford Acute Stress Response Questionnaire(SASRQ)to select the case group and the control group at 6to 10 days after the onset of the disease,and also meet the time 2 days later AMI patients romoved from the CCU,the acute stress response of the patients with acute myocardial infarction was to screened out the acute stress disorder group(n=138)and the non-acute stress disorder group(n=276).The self-designed social demographic data,Family APGAP Index,Social Support Scale,Pain Visual Analog Scale and part of Pittsburgh Sleep Scale were used to evaluate the general demographic data,treatment,cardiac function,the degree of pain,the degree of sleep,the situation of sleep,the status of family social support of acute myocardial infarction,and to compare the differences of disease status,treatment status,pain degree,sleep condition and family social support status between the two groups.2 All the data were using Excel software to establish the database and the SPSS17.0 statistical software was used to analyze the data.Descriptive statistical analysis was used to analysis the general demographic data;Chi-square test was used in the univariate analysis;and the Chi-square segmentation method was used to compare the two groups;and multivariate analysis was performed using binary logistic regression analysis.The meaningful factors based on univariate analysis were seleced as independent variables,and the acute stress disorder was seleced as the dependent variable for binary logistic regression analysis.Results 1 The univariate analysis of ASD in patients with acute myocardial infarction shows:age,marital status,educational level,place of residence,cohabitation,income,the number of cases,the number of illness,pain,Killip cardiac function,accepted rescue or not,treatment,the days in CCU,the number of coronary heart disease,sleep quality,departmental environmental impact sleep or not,the pain impact of sleep or not,the need for drug hypnosis or not,the difference was statistically significant(P<0.05).2Multivariate logistic regression analysis showed that the factors of ASD in patients with acute myocardial infarction are age,sex,Killip heart function,departmental environment affect sleep,pain affect sleep,defibrillation,lesion count,payment of medical expenses,social support level,family function is acute myocardial infarction in patients with acute stress disorder,poor heart function,the impact of sleep,pain affect sleep,defibrillation,multi-vessel disease,borrow money to pay medical expenses,family dysfunction are the risk factors for acute stress disorder in patients with cute myocardial infarction;and aged60 to 75 years old,high social support are the protective factors of acute stress disorder in patients with acute myocardial infarction.Conclusions The incidence of acute stress disorder in patients with acute myocardial infarction is related to the gender,the number of lesions and other conditions,chest pain symptoms,sleep status,family social support and other factors,and women,poor heart function,the department of environmental impact sleep,pain affect sleep,accepted defibrillation treatment,multi-vessel disease,borrowing money to pay medical expenses,family dysfunction are the risk factors for acute stress disorder in patients with acute myocardial infarction;and aged 60 to 75 years old,high social support are the protective factors of acute stress disorder in patients with acute myocardial infarction.
