Purpose As a life-threatening stressor,acute myocardial infarction can cause post-traumatic stress disorder,which not only has an adverse impact on mental health,but also leads to increased morbidity and mortality of cardiovascular diseases.The purpose of this study was to investigate post-traumatic stress disorder and its related influencing factors in patients with acute myocardial infarction,so as to provide reference for clinical psychological intervention in patients with acute myocardial infarction.Methods(1)Research objects: a cross-sectional study was conducted to analyze 300 inpatients with acute myocardial infarction admitted to the first department of cardiovascular medicine of shaanxi provincial people's hospital from September 2017 to June 2018.(2)Grouping: all subjects were graded using the Chinese civilian version of the PTSD symptom questionnaire.In this study,50 scores were used as the diagnostic threshold.Subjects with scores greater than or equal to 50 were divided into the PTSD group and those with scores less than 50 were divided into the non-ptsd group.(3)Data collection: general information of the included population was collected,including gender,place of residence,age,smoking index(smoking index = number of cigarettes per day × number of years of smoking),occupation,education level,etc.;Hemoglobin(HB),Plateletocrit(PCT),Mean platelet volume(MPV),platelet distribution width(PDW),Creatine kinase isoenzyme-mb,CK-MB),troponin I(Cardiac troponin I,c Tn I),Total protein,Total protein,TP),Albumin(Albumin,propagated),Total cholesterol,Plasma Total cholesterol,TC),Triglyceride(Plasma Triglyceride,TG),high-density lipoprotein cholesterol(High-density lipoprotein cholesterol,HDL-C),Apolipoprotein A(Apo A),Apolipoprotein B(Apo B),blood urea nitrogen(BUN),Serum creatinine(Scr),Retinol Binding Protein(RBP),etc.(4)SPSS19.0 statistical software was used for data analysis,including descriptive statistical analysis,t test,c2test,correlation analysis and Logistic regression analysis;Kolmogorov-smirnov was used for the normal test for each set of data.Measurement data conforming to the normal distribution were expressed as Mean ± standard deviation(` x ± s).Percentage was used for counting data,t test was used for measurement data between two groups,and c2test was used for counting data.Spearman correlation analysis was adopted to explore the correlation between PTSD scores and various factors in patients with acute myocardial infarction.Logistic regression analysis was used to explore the related factors of PTSD in patients with acute myocardial infarction.The test level was set as a=0.05,and P < 0.05 was considered as statistically significant difference.Results(1)Among the 300 patients with acute myocardial infarction,86 patients were diagnosed with post-traumatic stress disorder,with a prevalence rate of 28.7%,including 196 males with a prevalence rate of 22.4% and 106 females with a prevalence rate of 40.4%.124 cases were urban residents(21.8%)and 176 cases were rural residents(33.5%).(2)BUN of PTSD group and non-ptsd group were: 7.51 4.38(mmol/L)vs.5.525.99(mmol/L)(P < 0.05).PCT was 0.24 0.08(%)vs 0.22 0.07(%)(P < 0.05).CK-MB:79.94 97.51(U/L)vs 36.03 57.73(U/L)(P < 0.05).c Tn I were 15.12 18.74(ug/L)vs5.11 10.23(ug/L)(P < 0.05).TC: 5.06 1.13(mmol/L)vs.3.76 0.90(mmol/L)(P < 0.05).TG values were 1.89 1.07(mmol/L)vs.1.60 1.05(mmol/L)(P < 0.05).HDL-C was0.97 0.21(mmol/L)vs.1.14 0.31(mmol/L)(P < 0.05).Apo B were: 1.05 0.23(g/L)vs0.82 0.20(g/L)(P < 0.05).(3)Spearman correlation analysis: taking the positivity of PTSD as the dependent variable,and taking age,BUN,PCT,CK-MB,c Tn I,TC,TG,HDL-C,Apo B as the independent variable,Spearman correlation analysis was conducted.Age,BUN,PCT,CK-MB,c Tn I,TC,TG,HDL-C,Apo B showed a linear correlation with the occurrence of PTSD(P < 0.05).(4)Logistic regression analysis: positive PTSD was used as the dependent variable,and age,CK-MB,c Tn I,TC were the related factors of PTSD after adjusting for relevant factors(P < 0.05).Conclusions(1)The prevalence rate of post-traumatic stress disorder in patients with acute myocardial infarction is relatively high,28.7% in this study,among which the prevalence rate of male is 22.4% and female is 40.4%,which should be concerned.(2)Women,older,rural acute myocardial infarction patients are more likely to develop PTSD.(3)Age,CK-MB,c Tn I,TC were associated with post-traumatic stress disorder in patients with acute myocardial infarction.
