ObjectiveTo comprehend the level of rumination of patients with acute St-segment elevation myocardial infarction after percutaneous coronary intervention(PCI).To investigate the influence factors of rumination and the correlation between rumination and levels of hope and social support.The mediating effect of the level of hope between social support and rumination was further explored.To identify the specific populations of non-adaptive cognitive processing as well as the influencing factors of rumination.It will provide a reasonable and scientific theoretical basis for further research on the intervention of effective cognitive processing to promote the "double-heart health" of patients with acute STEMI after Percutaneous coronary intervention.Methods248 the acute STEMI patients who successfully completed PCI within 12 hours were selected as the subjects of this study in the department of cardiology of two tertiary general hospitals in a city of henan province from September 2018 to November 2019.The General Information Questionnaire,Social Support Rating Scale,Herth Hope Index,Chinese Event Related Rumination Inventory were used for the questionnaire survey.The sofeware of Epidata3.1 was used to establish the database and SPSS24.0 was used to analyze the data.The counting data were expressed by composition ratio and the measurement data were expressed by(mean ± standard deviation).One-way anova and T test were used in inter-group comparison.And multivariate linear stepwise regression was used in multivariate analysis.The Pearson correlation analysis method was used to analyze the correlation between the rumination and the social support and the levels of hope.Wen zhonglin's mediating effect was used to explore the mediating effect of the level of hope(mediating variable)between social support(dependent variable)and rumination(independent variable).Results1 The statistical analysis of 248 valid questionnaires collected show that the total score of rumination meditation was(34.73±5.080),in which the score of the dimension of purposive rumination was(18.31±3.093),and that of the dimension of intrusive rumination was(16.43±4.869).2 The statistical analysis of 248 valid questionnaires collected show that the total score of social support was(37.63±6.684),in which the subjective support score was(21.34±3.929),the objective support score was(8.06±3.102),and the support utilization score was(8.23±2.285).3 The statistical analysis of 248 valid questionnaires collected show that the total score of the hope level was(31.85±5.739),in which the score of positive attitude toward reality and future was(10.96±2.465),the score of positive behavior was(10.45±2.292),and the score of keeping close relationship with others was(10.44±2.092).4 The results of single factor analysis of rumination show that the factors influencing purposive rumination including age,religious belief,educational level,per capita family monthly income,and self-assessment of trauma(p<0.05).The factors influencing the invasive rumination including gender,per capita family monthly income,6min walking experiment results,self-assessment of trauma,and the number of negative lifestyle habits(p<0.05).5 The multiple linear stepwise regression analysis of rumination show that age,religious belief,educational level,per capita family monthly income,and self-assessment of trauma enter the regression equation,which were the relevant influential factors of the purposive rumination meditation.It can explain28.1% of the variation(R2=0.281).And the same time,gender,per capita family monthly income,6min walking experiment results,self-assessment of trauma,and the number of negative lifestyle habits enter the regression equation,which were the related influential factors of the invasive rumination meditation.It can explain 21.3% of the variation(R2=0.213).6 The Pearson correlation analysis results show: there was a positive correlation between purposive rumination and the social support and the hope level.(r = 0.414 ? 0.378,p<0.01).Intrusive rumination was negatively correlated with the social support and the hope level.(r=-0.402?-0.340?p<0.01).7 The analysis of the mediating effect of the hope level between the social support and rumination in the acute STEMI patients after PCI show that the hope level as the mediating variable partially mediates the relationship between social support and purposive rumination,with the mediating effect accounting for 27.6% of the total effect.At the same time,the hope level partially mediated therelationship between social support and invasive ruminative rumination,with the mediating effect accounting for 23.1% of the total effect.Conclusion1 Rumination was observed in the acute STEMI patients after PCI.The scores of rumination were on the high side of the medium.The scores of hope level and the social support are in the medium level.2 Rumination of the acute STEMI patients after PCI was influenced by demographic data(such as per capita family monthly income,gender,educational level,age,religious belief,etc.)and disease-related data(such as self-assessment of trauma,the number of negative lifestyle habits,six-minute walking experiment results,etc.).3 Purposive rumination of the acute STEMI patients after PCI was positively correlated with the hope level and social support,while invasive rumination was negatively correlated with the hope level and social support.The hope level had a partial mediating effect between social support and rumination.
