Objectives To analyze the influencing factors of acute stress disorder in patients after biliary tract surgery,so as to provide basis for taking countermeasures.Methods From October 2017 to December 2018,147 patients with acute stress disorder after biliary tract surgery were selected as case group and 147 patients with non-acute stress disorder underwent biliary tract surgery in Kailuan General Hospital of Tangshan City.One week after operation,the Stanford Acute Stress Response Questionnaire(SASRQ)was used to screen out the case group and the control group.147 patients with acute stress disorder after biliary tract surgery were included in the case group During the same period,147 patients with non-acute stress disorder underwent biliary surgery.The latter patients were the control group.Hospital Anxiety and Depression Scale(HADS),Perceived Social Support Scale(PSSS),Psychological Elasticity Scale(CD-RISC),Pain Visual Simulator Scale(VAS),Family Care Index Questionnaire(APGAR),and selfdesigned questionnaires on general conditions,disease,treatment and operation were used to evaluate the two groups.Excel software was used to establish the database,SPSS 17.0 was used to carry out statistics and analysis of data,chi-square test was used for single factor analysis,and multiple linear regression was used for multi-factor analysis.Results 1 Univariate analysis of acute stress disorder in patients after biliary tract surgery The results showed that the marital status,medical insurance type,personality characteristics,fasting time before operation,preoperative catheterization,preoperative chills and fever,preoperative pain,type of operation,degree of pain after operation,postoperative nausea,vomiting,abdominal distension,constipation,abdominal drainage tube,whether the drainage tube affects mood,whether the drainage tube affects sleep,whether or not.There were significant differences in fear,resilience,anxiety,depression,family function,social support level,difficulty in falling asleep,dyspnea,postoperative ward environment and surgical experience(P < 0.05).2 Multivariate Analysis of Acute Stress Disorder Affecting Biliary Postoperative Patients Multivariate logistic regression analysis showed that the types of medical insurance,personality,preoperative chills and fever,preoperative catheterization,postoperative pain,postoperative vomiting,constipation,abdominal drainage tube,whether the drainage tube affected mood,family support level,anxiety,depression,resilience,dyspnea and surgical experience were acute after biliary tract surgery.The influencing factors of stress disorder include introversion,preoperative chills and fever,preoperative catheterization,postoperative wound pain,postoperative vomiting,postoperative constipation,postoperative indwelling abdominal drainage tube,postoperative drainage tube affecting mood,postoperative dyspnea,anxiety,depression and surgical experience,which are the risk factors for ASD after biliary tract surgery.Medical insurance and good family function are the risk factors for ASD after biliary tract surgery.High level of resilience is a protective factor for ASD after biliary surgery.Conclusions The influencing factors of acute stress disorder in patients after biliary tract surgery are the type of medical insurance,personality,preoperative chills and fever,preoperative catheterization,postoperative pain,vomiting,constipation,drainage tube,whether the drainage tube affects mood,family support level,anxiety,depression,resilience,breathlessness and surgical experience.The influencing factors of acute stress disorder after operation include introversion,preoperative shivering and fever,preoperative catheterization,high degree of wound pain,postoperative vomiting,postoperative constipation,post-operative indwelling drainage tube,post-operative drainage tube affecting mood,post-operative dyspnea,anxiety,depression and surgical experience,which are the risk factors for ASD after biliary tract operation.Medical insurance,family and family are the risk factors for ASD after biliary tract operation.Good function and high level of resilience are protective factors for ASD after biliary surgery.Figure 0;Table 13;Reference 121.
