Aim: To Compare contrast visual acuity and visual acuity of all distance(including distance,intermediate and near visual outcome)between eyes implanted with Apodize ddiffractive multifocal IOL and Single focus intraocular lens under Different lighting conditions.Methods: Patients with age-related cataract who were to undergo phacoemulsification were divided into two groups: Apodize ddiffractive multifocal IOL(DMIOL group)vs Single focus intraocular lens(SIOL group).45 eyes with 45 patients were implanted with DMIOL,32 eyes with 32 patients were implanted with SIOL(SIOL group).Parameters analyzed included uncorrected distance visual acuity(UCDVA),uncorrected near visual acuity(UCNVA)and uncorrected intermediate visual acuity(40,60,100cm),and correted distance visual acuity(CDVA)Colen brander mixed contrast card set charts were recorded that were externally illuminated with 50 lux,250 lux and 500 lux and with different contrast which were 100%,25%,10% and 5%.The pupil diameter was measured.At the same time,two groups of patients were performed the visual quality questionnaire,and the scores were recorded at 1 and 3 monthspost operative respectively.Results:the total scoreaverage value of visual quality questionnaire 1 month postoperative between SIOL group and DMIOL group was respectively 49.12±2.86 and 51.32±3.12,the difference is statistically significant(t=0.876,P = 0.032);the total score average value of SIOL group and DMIOL group 3 months postoperative is respectively.On the 1 month and 3months postoperative,the quality of life satisfaction scores of two groups was no significant difference(P > 0.05).Whether in strong,medium or dark lighting conditions,with the declines in the contrast,the DMIOL group and the SIOL group of patients with postoperative vision,including UCDVA,UCNVA and CDVA are gradually decreased.Spearman rank correlation analysis results show that UCDVA,UCNVA and CDNV were significant positive correlation with background light intensity and contrast(r>0,P <0.05);.Whether in the weak,medium and strong lighting conditions,the mean of CS in DMIOL group were lower than SIOL group,but was no significant difference(P>0.05);under strong illumination,CS mean value in DMIOL group only in the high spatial frequency of 18 cpd,the CS value of DMIOL group was significantly lower than that of SIOL group(p<0.05).Conclusion: DMIOL can obtain all distance visual acuity and visual quality satisfaction.The UCDVA,UCNVA and CDNV were positively correlated with the light intensity and contrast.Under the conditions of illumination and contrast,UCDVA,UCNVA and CDNV in the DMIOL group were superior to those of the SIOL group.No matter what kind of lighting conditions,the CS value of DMIOL group was lower than the single focus group at each spatial frequency,especially in the high spatial frequency.
