Objective:To compare the visual function in eyes implanted with 3 different types of intraocular lens.(two different additional degrees of refraction of the AcrySof IQReSTOR diffractive multifocal intraocular lens,the monofocal aspherical intraocular lens AcrySoflQ),so as to evaluate the clinical application effect and the value of different additional refractive multifocal intraocular lens.It can provide a certain theoretical basis for selecting the clinical application of individualized intraocular lens.Methods:During the period from January 2015 to February 2016 in the Department of Ophthalmology of our hospital,20 patients(20eyes) who underwent phacoemulsification extraction combined with the monofocal aspherical intraocular lens AcrySofIQ(SN60WF) in implantation,40 patients(42 eyes)who underwent phacoemulsification extraction combined with the multifocal intraocular lens in implantation,23 patients(24 eyes multifocal intraocular lens implantation) AcrySof ReSTOR IOL 3.0D (SN6AD1),17 cases (18 eyes) implantation of multifocal intraocular lens AcrySof ReSTOR IOL 2.5D (SV25T0).Postoperative follow-up review after three months,using slit lamp microscope to observe the eye recovery of intraocular lens implantation.We also evaluated the spectacle dependence,unwanted visual symptoms and patient satisfaction.3 months after surgery.we recorded:uncorrected and best corrected distance visual acuity,uncorrected and best corrected near visual acuity,distance corrected intermediat(40cm, 60cm and 80cm) and near visual acuity.the IOL eye pseudoaccommodation; modulation transfer function function analyzer checks drawn out 5,10,15,20,25, and 30cpd of spatial frequency,strehl ratio,wavefront aberration. The iTrace analysis modulation transfer function,strehl ratio and wavefront aberration under different pupil diameters after implantating three intraocular lens in patients.Results:1. Postoperative IOL basic situation:the pupil had normal activitiy in all patients. IOLs are located in the capsular bag.There were no posterior capsular opacification and other complications.2. Three months after surgery,the the rate of spectacle dependence in daily life are:IQ 35.0%(7/20),+2.5D 77.8%(14/18),+3.0D 83.3%(20/24) respectively.The group of IQ had 2 patients complained day light halo,4 patients had night glare phenomenon.The group of+2.5D had 2 patients complained day light halo, 5 patients had night glare phenomenon.The group of+3.0D had 4 patients complained day light halo,6 patients have night glare phenomenon.3. The uncorrected and best corrected distance visual acuity,and best corrected near visual did not show significantly statistical differences(P> 0.05).The uncorrected near visual acuity,distance corrected intermediate (40cm,60cm and 80cm) and near visual acuity.the difference was statistically significant (P< 0.05).4. The amplitude of accommodation of IOL:The difference is statistically significant between the groups(P< 0.05):IQ<+2.5D<+3.0D.5.Modulation Transfer Function and strehl ratio:In different pupil diameter,there was not statistically significant between three groups(P>0.05).6.Wavefront aberration:There was not statistically significant between three groups(P>0.05).Conclusions:1. Compared with the monofocal aspheric intraocular lens, two kinds of multifocal intraocular lens significantly reduced the rate of spectacle dependence for patients.It’s available to meet different requirements of visual function in patients with two kinds of multifocal intraocular lens in the differences of intermediat and near visual acuity by the differences of design features.The performance for near visual acuity of multifocal intraocular lens +2.5D is better than the other two kinds of intraocular lens (IOL).But in the intermediat distance vision,the patients with implantation of multifocal intraocular lens+2.5D will be more comfortable.2.Comparison of visual quality after implantation of two different multifocal intraocular lens by iTrace.It objectively explains the reasons for the low incidence of postoperative visual symptoms and high satisfaction of patients.The various functional indicators of iTrace can evaluate visual quality very well.
