Part one: literature analysis of the curative effect of traditional Chinese medicine in the treatment of unstable angina pectorisObjective:To evaluate the curative effect evaluation index of traditional Chinese medicine for the treatment of unstable angina pectoris and analyze the application and problems of various indexes.The initial set of clinical efficacy evaluation of traditional Chinese medicine in the treatment of unstable angina pectoris was established,which provided the basis for the construction of the end point of clinical curative effect evaluation of unstable angina pectoris.Method:Using computer-based Chinese medicine for the treatment of unstable angina and published in the core journals of randomized controlled clinical research literature,the search tools are Chinese Biomedical Literature Database(CBM),Chinese Academic Library(CNKI),Chinese Science and Technology Journal Database(VIP),published from January 1,2005 to December 31,2014.Screening out the standards of the literature.To extract the information of literature and to make a literature survey on the evaluation index of unstable angina pectoris.Statistical analysis of the selection and use of various indicators by means of statistical methods.Result:220 articles were screened,and the evaluation indexes of curative effect in 220 articles were classified,The clinical symptoms(94.55%),physical and chemical indexes(95.45%),end point index(18.18%),quality of life index(2.73%)and safety index(47.27%)were included in this study.Most of the literatures were mainly based on physical and chemical indexes and clinical symptoms.However,the choice of endpoint events and quality of life indicators were less.The highest frequency of use of the indicators for the physical and chemical indicators combined with the symptoms of indicators and safety indicators(37.27%),followed by physical and chemical indicators and clinical symptoms of the combination of the indicators(28.18%).Conclusion:At present,the evaluation index of traditional Chinese medicine mainly includes:Physical and chemical indicators,clinical symptoms indicators,quality of life indicators,safety indicators,including the end of the year in recent years,attention to the indicators,we can see the application of the evaluation of the effectiveness of the indicators of diversity.In the choice of specific indicators,most of the literature to physical and chemical indicators and clinical symptoms of the main indicators.Lack of evidence of end-point events in recent years.Part two:The end-point index of clinical efficacy evaluation of TCM in the treatment of unstable angina pectoris during hospitalizationObjective:To investigate the incidence of end-point events in patients with unstable angina pectoris in Shandong Provincial Traditional Chinese Medical Hospital,and to calculate the weight of each index by principal component analysis and frequency statistics.To establish the end point of clinical efficacy evaluation of unstable angina pectoris for unstable angina Evaluation of efficacy to provide a scientific basis.Method:Production of "Chinese medicine hospital unstable angina patients cohort study questionnaire".Using a medical record.Through the medical record information management system,the hospital affiliated to Shandong University of Traditional Chinese Medicine was selected from January 1,2008 to June 30,2015.The general data of the patients and the occurrence of the end point of hospitalization were collected.Epidata 3.02 software was used to establish the information database of patients with unstable angina.Export to Excel 2003 and SPSS 17.0 software package,use the principal component analysis method to calculate the weight of the end point of hospitalization,statistics of the end point of the occurrence of frequency,as the frequency weight,and then use the multiplier synthesis method to calculate the combined weight of the two,combined Clinical significance,the establishment of clinical efficacy of traditional Chinese medicine evaluation of the end of the indicators.Result:The weight of each index during hospitalization end point were revascularization(0.299),non fatal myocardial infarction(0.286),non fatal stroke(0.277),acute heart failure(0.103),severe arrhythmia(0.013),cardiac death(0.010),bleeding(0.007),acute renal failure(0.006 etc.).Conclusion:According to the clinical significance and the intervention of traditional Chinese medicine sensitivity,get the main evaluation index during hospitalization for cardiac death,nonfatal myocardial infarction,nonfatal stroke,secondary evaluation index for acute heart failure,severe arrhythmia,revascularization.Part three:The end-point index of clinical efficacy evaluation of TCM in patients with unstable angina pectoris during follow-upObjective:To investigate the incidence of end-point events during the follow-up period of patients with unstable angina pectoris in Shandong Provincial Traditional Chinese Medical Hospital,and to calculate the weight of each index by principal component analysis and frequency statistics.To establish the end point of clinical efficacy evaluation of unstable angina pectoris for unstable angina Evaluation of efficacy to provide a scientific basis.Method:Follow-up survey.The patients were followed up for the time being after discharge to June 30,2015.The incidence of patient end points during follow-up was collected.Create an information database,enter data.The incidence of 3 months,6 months,1 year,2 years,4 years after the onset of the disease.Using the principal component analysis method to calculate the weight of each index,And then use the method of geometric mean to calculate the five principal hours of each component of the weight of the composition of the weight;the same time to obtain the five time points of the comprehensive frequency weight;Finally,the use of multiplier synthesis method to calculate the five time indicators of the comprehensive weight,Combined with clinical significance,the establishment of clinical efficacy evaluation of traditional Chinese medicine indicators.Result:The comprehensive weight of each index during follow-up were angina and readmission(0.795),revascularization(0.049),cardiovascular readmission(0.033),acute heart failure(0.031),(0.028)cardiac death,non fatal myocardial infarction(0.025),severe arrhythmia(0.011),non fatal brain stroke(0.009).Conclusion:During the same period of time,the importance of each indicator was different during the follow-up period;there was a difference in the importance of the same indicator at different times.Combined with clinical significance and the sensitivity of Chinese medicine intervention,the follow-up period of the main end point for cardiac death,non-fatal myocardial infarction,secondary endpoint for angina re-admission,revascularization,acute heart failure.
