Primary Breast Cancer With Thyroid Cancer Patients Clinical Characteristics Analysis

[objective] :the study by analyzing the clinical characteristics of patients with primary breast cancer with thyroid cancer, risk factors and prognosis, for two kinds of cancer prevention and control to provide certain theoretical basis.[methods]:collect the tumor hospital of yunnan province in January 2006 to December 2016 were merged thyroid cancer cases,106 cases of breast cancer,including breast cancer after (B1T) in 37 cases with thyroid cancer, thyroid cancer after breast cancer (T1B) in 48 cases, at the same time (BT) 21 cases with breast cancer and thyroid cancer,and according to the scale of 1:4 and BIT and TIB group of breast cancer were randomly collected in the same period with no merger (oB), 148 cases of thyroid cancer, only (oT), 192 cases of thyroid carcinoma. Analysis between groups of patients with the clinical characteristics,risk factors and prognosis,and the related data for statistical analysis.[results]:(1)general characteristics:patients in group B1T mean age:47.69 +/-9.54 years, with "40-50 age group accounts for the largest proportion, accounting for 43.2% of the total, oB group of patients with an average age of:46.83 +/- 9.52, the same to 40-50 age group accounts for the largest proportion,accounting for 38.5% of the total, the collected oT group of patients, mean age: 45.74 +/- 9.32 years, with" 30 to 40 years old age group accounts for the largest proportion, accounting for 31.3% of the total.The 48 cases collected TIB group of patients, average age: 46.33 +/- 9.66 years old, of which the 50 to 60 years old age group accounts for the largest proportion, 33.3% of the total.Second primary lung cancer, the average time interval B1T group than T1B short (3.56+3.23 vs5.72 +/- 3.17 years), the difference was statistically significant (p<0.001).(2) the pathological characteristics: TIB and oT group two groups of thyroid cancer pathological types are the most proportion of papillary carcinoma (PTC), TIB group:85.4%, oT group: 88%, followed by a follicular carcinoma (for TIB group, 4.2% in oT group: 6.3%).BT group and oB pathological type of breast cancer in both groups were given priority to with infiltrating ductal carcinoma (BT group: 81.1%, oB groups:81.1%), followed by intraductal carcinoma (BT group: 10.8%, and oB group; 12.1%),molecular classification is given priority to with LuminalB (BT group; 51.4%, and oB group; 52%).The expression of ER and PR BIT group was obviously higher than that of oB group (ER + 75.7% than 64.9%, PR + 73.0% versus 61.5%), the difference was statistically significant (p < 0.05), mean Ki - 67 B1T groups compared with oB group was higher than 15 (30), the difference was statistically significant (p < 0.05).Thyroid function (3) : in the group of BIT, TIB and BT group of FT3, FT4, TSH,TGAb, TPOAb in comparison, there was no statistically significant difference (p>0.05).T1B compared group and oT group, FT3, FT4 value comparison, there was no statistically significant difference, (p > 0.05), but and TGAb TPOAb comparison, TIB group is higher than the oT group (TGAb: 5.28 than 5.28, TPOAb: 2.63 3.21), the difference was statistically significant (p<0.05).(4) the prognosis: BIT, BT group and oB group average OS is respectively 110112 and 118 months, five years OS rate are 88.8%, 88.8% and 88.8% respectively, and 10 years OS rate were 69.7%, 73.1% and 78.5%, B1T group compared with BT group,there was no statistically significant difference survival time (p > 0.05), but BIT group compared with oB group, survival time is shorter, the difference was statistically significant (p<0.05).(5) other: BT group (37 cases) and oB group (148 cases), compared to the diagnosis of breast cancer's age, menopausal status, family history, pathological type,histological grade, tumor size, lymph node metastasis, distant metastasis, molecular classification, whether breast cancer postoperative radiotherapy, comparison between the two groups, there was no statistically significant difference (p>0.05).TIB 48 patients and oT group 192 patients clinical characteristics to compare average onset age,family history,pathological type,tumor size,lymph node metastasis,distant metastasis, the proportion of multiple lesions are similar, there was no statistically significant difference (p>0.05).[conclusion]1,the first primary lung cancer onset age in breast cancer in thyroid cancer merger with "40-50" scale most, in the age at first in patients diagnosed with breast cancer after 3-4 years than any other age group to strengthen the inspection of thyroid, thyroid cancer in breast cancer merger to scale up 50 to 60 years old, in the same way,in this age of thyroid carcinoma in 4 to 5 years after the need to pay close attention to the occurrence of mammary gland disease.2, ER, PR in breast cancer patients with positive may indicate a higher risk of thyroid cancer, Ki - 67 high expression may increase breast cancer risk of thyroid cancer again.3, thyroid cancer patients should pay more attention to TGAb, TPOAb increase, may be increase the risk of breast cancer.4, although thyroid cancer prognosis is good, but suffer from thyroid cancer after breast cancer increases the risk of breast cancer death.
