The TCM Syndrome Type Distribution And Analysis Of Influence Factors In MHD Patients With Insomnia

Objective:By cross-sectional survey,research and discuss TCM syndrome type distribution in maintenance hemodialysis(MHD)patients with insomnia,establish its primary symptom.And discuss the possible influencing factors by related analysis,to provide better guidance for the MHD patients with insomnia in traditional Chinese medicine treatment.Method: Basic information and the Pittsburgh sleep quality index(PSQI)were investigated of 273 patients with stable hemodialysis,according to the inclusion criteria 273 patients were divided into insomnia and insomnia group.By two sample meant test,analyze the age,gender,age of dialysis,biochemical examination in the two groups.For the factors that P<0.05,adopt the two indexes classification logistic regression analysis,to obtain the influence factors of insomnia.Insomnia patients fill in MHD patients with insomnia syndromes questionnaires,after finishing check,converse the binomial distribution of the variables in the questionnaire into numerical data.Set up insomnia syndromes database table in MHD patients with EXCEL2007.Cut diagnostic information using frequency analysis and get common factor after factor analysis.By clustering analysis to gain the distribution regularity of TCM syndrome types.Result: Above-mentioned MHD patients have a total of 109 patients with insomnia,morbidity was 39.92%.By the logistic regression analysis,PTH is the influence factors of MHD patients with insomnia.Frequency of syndromes analysisshowed that mouth throat dry,itchy skin,difficulty sleeping,lumbar debility,constipation,body weight,anorexia,languid,etc are the 49 high-frequency syndrome,and then get 19 common factor after factor analysis.Cluster analysis showed that when the Euclidean distance from 17,these factors can be divided into six classes.Refer to standard TCM disease diagnosis and advice of expert,sum up the following six type: Yang insufficiency of both the spleen and the kidney,liver depression transforming into fire,retention of phlegm and blood stasis,deficiency of both heart and spleen,Yin deficiency and fire hyperactivity,heart-gallbladder-qi deficiency.Conclusion:PTH as a kind of uremic toxins directly acts on central and peripheral nervous system which causes insomnia.Yang insufficiency of both the spleen and the kidney,retention of phlegm and blood stasis,are seldom mentioned in available literature about the general population with insomnia.Factors are closely related to the mixture of efficiency and excess pathogenesis.Weakened body resistance is the cause,pathogenic factor is the symptom.Weakened body resistance gives priority to Spleen and kidney Yang deficiency,coxcombry Yang qi is difficult to enter into Yin,since it could cause insomnia.Pathogenic factoris particularly illustrated in phlegm and blood stasis,which influence the run of defensive qi that remain in the Yang,not into the Yin.The above-mentioned mechanism can also cause insomnia.
