Study On The Correlation Between Traditional Chinese Medicine Syndrome Types And Biochemical Indicators In Patients With Maintenance Hemodialysis Combined With Sleep Disorders

Objective:To evaluate the sleep quality of patients with maintenance hemodialysis(MHD)and the characteristics of TCM syndrome types,to explore the correlation between the common biochemical indicators affecting the sleep quality of MHD patients(such as hemoglobin,serum albumin,hypersensitive C-reactive protein,whole-segment parathyroid hormone,and blood phosphorus)and the TCM syndrome types,and to improve the research basis for the treatment of MHD combined with sleep disorders from the perspective of sleeplessness,It also provides a theoretical basis for the clinical treatment of MHD combined with sleep disorders by combining traditional Chinese and western medicine.And according to its correlation,it can promote the TCM syndrome differentiation of sleep disorders in MHD patients to develop in a more rational and standardized direction,highlighting the advantages of integrated diagnosis and treatment of traditional Chinese and western medicine.Methods:During the period from June 2021 to June 2022,patients undergoing maintenance hemodialysis treatment in the hemodialysis room of Ruikang Hospital affiliated to Guangxi University of Traditional Chinese Medicine first used the PSQI score to assess the sleep quality of patients,and then used the scores of the Self-rating Anxiety Scale and Hamilton Depression Scale to exclude MHD patients with sleep disorders caused by anxiety and depression,and obtained 139 patients with maintenance hemodialysis combined with sleep disorders who met the inclusion criteria,Collect the general clinical data and relevant biochemical indicators affecting sleep quality,including hemoglobin,serum albumin,hypersensitive C-reactive protein,whole parathyroid hormone and blood phosphorus;According to the TCM syndrome differentiation criteria of "insomnia",MHD patients with sleep disorders were divided into five types,namely,liver-fire disturbs the heart,phlegm-heat disturbs the heart,heart-spleen deficiency,heart-kidney disjunction,and heart-blood-qi deficiency,using the TCM syndrome integral scale and the patient’s symptoms,physical signs,tongue and pulse.The database was established and statistically scanned and analyzed with the support of SPSS22.0 system.Result:1.MHD patients with sleep disorders were divided into 26 cases(18.7%)of liver-fire disturbing heart type,28 cases(20.1%)of phlegm-heat disturbing heart type,40 cases(28.8%)of heart-spleen deficiency type,18cases(12.9%)of heart-kidney disjunction type,and 27 cases(19.5%)of heart-blood-qi deficiency type.2.Analyze the dialysis age of MHD patients with sleep disorders,and draw a conclusion that there is a significant correlation between TCM syndrome types and dialysis age(p=0.00<0.05),of which the liver fire disturbs the heart syndrome has a large correlation,and the heart and spleen deficiency syndrome has a small correlation.3.By analyzing the relevant biochemical indicators that affect MHD combined with sleep disorder in this study,we can see that there is a significant difference between the TCM syndrome types for the whole segment of parathyroid hormone(i PTH)(p=0.000 < 0.05),suggesting that it has the greatest correlation with the syndrome of liver fire disturbing the heart and the smallest correlation with the syndrome of heart and gallbladder qi deficiency;There was a significant difference in blood phosphorus(Pi)between TCM syndrome types(p=0.000 < 0.05),suggesting that it had the greatest correlation with liver fire disturbing heart syndrome and the smallest correlation with heart and gallbladder qi deficiency syndrome;There was a significant difference in serum albumin(ALB)between TCM syndrome types(p=0.000 < 0.05),suggesting that ALB was decreased,heart and kidney syndrome was not obvious,ALB was increased,and heart and gallbladder deficiency syndrome was significant;TCM syndrome types showed significant differences in hemoglobin(Hb)(p=0.000 < 0.05),suggesting that Hb decreased,heart and spleen deficiency syndrome was significant,Hb increased,and phlegm-heat disturbing heart syndrome was significant.There is no obvious correlation between TCM syndrome types and hypersensitive C-reactive protein(hs-CRP).Conclusion:1.Among the TCM syndrome types of maintenance hemodialysis combined with sleep disorder,the heart and spleen deficiency syndrome is the most common,followed by phlegm-heat disturbing heart syndrome,liver-fire disturbing heart syndrome,heart-blood-qi deficiency syndrome,and heart and kidney failure syndrome.2.There is no significant correlation between TCM syndrome types and gender,age and PSQI.3.The correlation analysis between TCM syndrome types and dialysis age suggests that with the increase of dialysis age,the incidence of MHD patients with sleep disorders is higher in liver fire disturbing heart syndrome.4.Correlation analysis between traditional Chinese medicine syndrome types and biochemical indicators,indicating;The increase in i PTH and Pi values is evident in MHD patients with sleep disorders,as evidenced by liver fire disturbance;ALB is reduced,and patients with MHD combined with sleep disorders have significant heart kidney disharmony;Hb decreases,and patients with MHD combined with sleep disorders exhibit significant deficiency in both the heart and spleen.There is no correlation between traditional Chinese medicine syndrome types and hs-CRP.
