Study On The Correlation Between Mental State And TCM Syndromes In Maintenance Hemodialysis Patients

Purpose:Assess the status of mental health of patients with MHD, screen the biochemical index that effects emotion, analyze the relationship of mental treatment and TCM syndrome types, then provide certain evidence for early discovery and prevention and TCM treatment theory of mental disease.Methods:Using cross-sectional epidemiological investigation method of patients with maintenance hemodialysis, investigating the questionnaire of Self Rating Anxiety Scale and Self-Rating Depression Scale (SAS, SDS), Five-pattern Personality Inventory, chronic symptoms of kidney failure classification quantization table. Then get the scores of patients’ anxiety and depression,five-pattern personality, classification of TCM syndromes, HRV and vice sympathetic nerve strength, using the SPSS statistics analyze the relevance of the psychological state of patients with syndrome of traditional Chinese medicine.Results:In 140 syndrome types of patients, the most is the deficiency of both qi and yin deficiency syndromes in 36 cases (25.7%), followed by the spleen and kidney qi deficiency (23.5%), deficiency of spleen and kidney yang deficiency of yin and Yang (20%), the deficiency of yin and yang (17.1%), yin deficiency of liver and kidney (13.7%). In the empirical types of evil, the most is the dampness of 62 cases (44.2%), followed by damp heat in 42 cases (30%),32 cases (22.9%)of blood stasis and air in 4 cases (2.9%).The number of patients in state of anxiety is 24 people (17.1%); 50 patients with depression (35.7%), the scores of people with anxiety depression are higher than the healthy relatively. Women have higher anxiety depressive tendencies. Effects of PTH on anxiety and depression have difference, gender difference in scores of depression and anxiety has statistical significance, no significant difference in five -pattern personality scores of dialysis duration、primary disease、age and gender,no significant statistical differences in anxiety and depression scores of biochemical, blood routine index、primary disease、age and dialysis duration, no significant statistical differences of the distribution of TCM Syndromes in biochemical index、gender and age, the TCM syndromes of different dialysis duration and primary disease have statistical significant differences.The main TCM syndromes in which the dialysis duration is less than 3 years of hemodialysis patients are deficiency of both qi and Yin and yang deficiency of spleen and kidney. The main TCM syndromes in which the dialysis duration is greater than 3 years in patients are deficiency of both qi and Yin and spleen qi deficiency. The main TCM syndromes of chronic glomerulonephritis are spleen and kidney qi deficiency and qi and yin deficiency, and the main TCM syndromes of chronic interstitial nephritis are Yang deficiency and the main TCM syndromes of diabetic nephropathy are yang deficiency of spleen and kidney, yin and yang deficiency, qi and yin deficiency, primary disease of hypertension mainly is spleen and kidney qi deficiency. In five pattern personality, the scores of Tai yang, Shao yang, yin-yang balanced and Shao yin are higher than the national norm, the score of Tai yin is lower than the national norm. The scores of Tai yang in deficiency of qi-yin and yin-yang syndrome type are higher, The scores of Shao yang in liver-kidney yin deficiency syndrome type are higher.The Shao yin and Tai yin scores in yin-yang deficiency are higher. In damp heat and wind syndrome,Tai yang scores are higher.the scores of shao yin in wet muddy syndrome are higher. The anxiety scores in Yang deficiency of spleen and kidney and deficiency of qi-Yin Syndrome are higher than the other syndromes.The depression scores in blood stasis and damp heat syndrome are higher than the other syndromes. The scores of Shao yang and Shao yin and tai yang in anxiety and depression patients and non anxiety and depression patients have no significant difference, anxiety and depression in patients with Yin Yang and score lower than non anxiety depression. The scores of yin-yang balanced in anxiety and depression patients are lower than non anxiety depression,while tai yin higher.Comparison of five pattern personality in anxiety and depression patients and non anxiety and depression patients, yin-yang balanced and Tai yin scores have difference. The heart rate variability of 97 cases in dialysis patients were lower than the normal reference value, but there was no significant difference. The parasympathetic nerve strength of anxiety and depression patients is lower than the non, but the difference was not statistically significant.The anxiety scores are negatively related with yin-yang and tai yin, and the depression scores are positively related with tai yin.Conclusion:1. Women and high PTH level are more vulnerable to anxiety and depression, so female patients and high PTH level should get more protection of anxiety and depression.2.The number of tai yang personality patients with qi-yin and yin-yang deficiency syndrome is higher.The same is the number of Shao yang personality patients with yin deficiency of liver and kidney, the number of Shao yin personality with qi deficiency of spleen and kidney, the number of shao yin and Tai yin personality with deficiency of yin-yang, the number of tai yang personality with heat and wind, the number of shao yin personality with deficiency of turbid dampness.3.The number of anxiety patients with yang deficiency of spleen and kidney and deficiency of qi-yin is higher.The number of depression patients with blood stasis and damp heat syndrome is higher. The number of tai yin personality in anxiety and depression patients is higher than the non. in and yang deficiency and tai yin personality is not easy to be anxiety, Yin-yang balanced and tai yin personality is inclined to be depressed.
