Objective:To investigate the effect of intraoperative infusion of modified polarized fluid(GIK)on postoperative insulin resistance in patients undergoing radical colorectal cancer surgery to achieve the purpose of further optimizing the perioperative liquid therapy regimen.[Methods]:40 cases of colorectal cancer underwent radical anesthesia,ASA grade I??,aged 50 to 70 years old,weighing 40?70kg,previous without diabetes,severe cardiovascular and cerebrovascular diseases,intestinal obstruction,gout,moderate to severe anemia And other complications,liver and kidney function is normal.Preoperative diet for 8 to 12 hours,the operation time of 2.5 to 5.0 hours.The patients were randomly divided into two groups:I conventional control group(n= 20);?experimental group:modified polarized fluid intervention group(n = 20);J group of patients with intraoperative infusion of compound electrolyte and hydroxyethyl starch solution,(GIK)and hydroxyethyl starch were used in the infusion of group ?,and the ratio of gel to rubber was 2:1.The fasting serum insulin(FINS),fasting blood glucose(FPG)and C-peptide were recorded in the early morning(TO)and the first day after the operation(T1).The operation time,blood loss and NI were recorded.Record the patient's first anal exhaust time,the first time to get out of bed,the total length of stay.[Results]There were no significant differences between the two groups in terms of gender,age and body mass index(P>0.05).There was no significant difference between the two groups in terms of NI value,operation time,blood loss and postoperative VAS score(P>0.05).Four indicators included fasting blood glucose,fasting insulin,insulin resistance index,C peptide four indicators of the group were compared:whether the control group or the experimental group,compared with preoperative,after the four indicators were increased(P<0.05(P<0.05).The results showed that the four groups of the experimental group were significantly different from the control group(P<0.05),and the control group was significantly different from the control group(P<0.05).The time of first expiratory exercise was shorter in the experimental group than in the control group(P<0.05).(Conclusion]The modified GIK solution was used to improve the postoperative insulin resistance,reduce the probability of insulin resistance,shorten the number of hospital days and save medical resources.
