Objective:High ligation of hernia is a classic surgical treatment of indirect inguinal hernia in children.The traditional small abdominal transverse incision for pediatric indirect inguinal hernia is a mature technology,but the problem that scrotal swelling after,recurrence,contralateral new hernia complications is of common after surgery.Laparoscopic technique in the treatment of pediatric inguinal hernia is becoming mature,and because of its simple operation,and it could find lateral latent hernia and low incidence of scrotal swelling,it's becoming welcome in families of children and in industry.This study was to explore the single port laparoscopic and conventional abdominal transverse small incision surgery in the treatment of pediatric inguinal hernia treatment,and discuss the feasibility of clinical application.Methods: A retrospective analysis in June 2014 to June 2016 year including 356 cases of indirect inguinal hernia in children was studied.According to the way of operation,the children were divided into two groups,including 302 cases of laparoscopic surgery(laparoscopic group),54 cases with abdominal transverse incision surgery(traditional surgery group).The surgery of laparoscopic group was operated by the same operater with a special needle to complete the percutaneous puncture of the inner loop of the ligation of the inner loop.The curative effect and complications of the two kinds of operation were compared.All patients were followed up after operation,the collected data including operative time,postoperative hospitalization time,perioperative complications,recurrence,with postoperative iatrogenic cryptorchidism or testicular atrophy.Results : There were 43 cases of latent hernia were found in 262 patients unilateral inguinal which were managed in same time.The average time of operation laparoscopic group,traditional surgery group,was 14.5 ± 3.1min,34.2 ± 4.4min,P<0.05,bilateral operation time was 22.7 ± 7.9min ? 46.3+6.7min P<0.05;In laparoscopic group,there were 3 patients of postoperative swelling of the scrotum,and no cases with scrotal,hematoma and iatrogenic cryptorchidism,2 patients of recurrence in the follow-up;There were 2 patients of linear reaction after operation.In traditional operation group,there were 22 patients of postoperative swelling of the scrotum,5 patients with scrotal hematoma,1 patients of homolateral and 10 patients of contralateral recurrence in the follow-up.Conclusion : 1.Compared with traditional small incision surgery,laparoscopic procedure is superior in the beauty of incision,lower rate of postoperative complications and contralateral recurrence.The laparoscopic herniotomy is a safe,reliable and effective option for the treatment of pediatric indirect inguinal hernia.2.Single port laparoscopic herniotomy which use a special hernia needle through the skin of abdominal wall to the inner loop ligation,the ligation line can be basically maintained at the same path,to ensure that no tension in the peritoneal clearance of close ligation,Contralateral occult hemia can be detected and ligated intraoperatively,which has its unique advantages for the treatment of children with recurrent hernia,hernia and hydrocele of the recessive.
