Floating Shoulder Injury Classification And Treatment Options

Objective: Through the analysis of clinical efficacy between non-operation and operation treatment in floating shoulder injury to explore the floating shoulder injury treatment options in order to guide clinical practices.Materials and Methods: 57 cases with floating shoulder injury that had been admitted to orthopedics in our hospital from January 1990 to December 2015 were retrospectively reviewed.In combination with Hardegger classification and Hecscovici classification : the combined scapular neck fracture of clavicle fracture in 20 cases,A broken collarbone combined scapular fracture in 12 cases,Scapular neck fracture merge the acromioclavicular ligament damage in 4 cases,the Coracoid process fracture merge clavicle fracture in 6 cases,the in Scapular fracture merge the acromioclavicular ligament damage 1 cases,the Clavicle fracture merge beak lock ligament damage in 4 cases,the in Merger scapular neck fracture of clavicle fracture beak shoulder ligament damage 2 cases,the in The acromioclavicular ligament damage coronoid process fractures 2 cases,the Pure scapular neck fracture in 6 cases.Off all 57 patients,14 cases were treated non-operation,43 cases were trested operation.Hardegger function evaluation system was adopted to rating the clinical efficacy of the two treatments.And the complications between two treatments were compared.We did the statistics analysis with 2? test by SPSS 17.0 sofeware.P <0.05 has statistic significance.Results: All cases were followed up for 2 months to 18 months,an average of 12 months.Among 14 patients treated by non-operation,Showed that the result were excellent in 4 cases,good in 6 cases,fair in 3 and poor in 1.Of 43 patients treated by operation,showed that the result were excellent in 30 cases,good in 7 cases,fair in 5 and poor in 3.There was no significant difference in excellent and good rate of the two groups(2? =0.117,P =0.732).Complications appeared in 10 cases of the non-operation,6 limited outreach,2 traumatic joint disease,1 joint instability,1 heterotopic ossification.Only 15 cases were appeared complications of the operation,9 limited outreach,3 traumatic joint disease,3 heterotopic ossification.There was significant difference between complications rate in the two groups(2? =4.700,P =0.030),non-operation are more complications than some by operation treatment.Conclusion: There was no significant difference in excellent and good rate of the non-operation and operation treatment.They all have been good clinical efficicy.Fore the severely displaced floating shoulder injury,operation treatment can achieve excellent reduction and fixation,but also reduce the complications compared with non-operation.The floating shoulder injury treatment program determined by the fracture location and displacement.It's very important to strictly control the indications for surgery.
