| Objective This study used Jianpi Yishen Huazhuo therapy for syndromes of spleen and kidney deficiency and phlegm in patients with hypothyroidism Hashimoto,through the observation of thyroid antibodies in the patients before and after treatment,thyroid function,blood lipid,clinical symptoms and degree of goiter,understand the effect of this disease,provide new methods and ideas for the prevention and treatment of this disease in the future.Methods1.A total of 64 patients with in the Department of endocrinology of Yunnan Provincial Hospital of traditional Chinese medicine from December 2015 to--2016,and the Department of traditional Chinese medicine of the people's Hospital of Yunnan Province in September were selected.They were randomly divided into treatment group and control group with 32 cases in each group.2.medication methods:(1)The control group: the use of conventional dose therapy(according to the specific circumstances of the patient,under the guidance of a doctor,the starting dose for daily use of 1/4 tablets,gradually increase the dose to 1/2 tablets,interval 2-4weeks,the maximum increase to 1-2 tablets per day).(Product Name: euthyrox(Euthyrox);50 g*100;Approval Number: registration certificate number H20100523;German Merck Co).(2)The treatment group: the routine use of Levothyroxine Sodium Tablets(the same as the control group)on the basis of traditional Chinese medicine Erxian Shencao decoction,one dose daily.Erxian ginseng grass soup dosage: each agent cold fried 400 ml,decoction 20 min,three clothes,a daily agent.3.Course of treatment: two groups were treated for a course of treatment for a total of4 weeks,a total of two courses of treatment.4.The main contents of the study were as follows: the changes of thyroid related immunological indexes,thyroid hormone levels,blood lipids,thyroid ultrasound and symptoms before and after treatment.Results1.A total of 64 cases of disease belongs to deficiency of spleen and kidney and phlegm syndrome in patients with hypothyroidism and Hashimoto's thyroiditis.11 cases were male(17.19%),53 cases were female(82.81%),male to female ratio is about 1:4.8;at the age of 16-75 years old,average 54.97±15.95 years.2.Before treatment,the two groups of age,gender,level of thyroid hormone and thyroid related immune indexes,blood lipids,thyroid enlargement,symptoms and curative effect(49)>0.05 statistical treatment,no significant difference,comparable.3.(1)In the reduction of thyroid antibody: the treatment group before and after treatment were significantly different((49)< 0.01),the control group before and after treatment was not statistically significant((49)> 0.05).After treatment,the comparison between the two groups was(49)< 0.01,which showed that the treatment group had a significant effect on reducing thyroid antibody compared with the control group.(2)On the improvement of thyroid function: the treatment group after treatment,FT3 and FT4 levels were significantly higher than before treatment((49)<0.01),after treatment,the level of TSH was significantly decreased compared with before treatment((49)<0.01).The treatment group can significantly improve the thyroid function.The control group after treatment,FT3 and FT4 were increased,FT4 increased significantly,(49)<0.01,and FT3 increased significantly,P<0.05,TSH level was significantly lower than before treatment,(49)<0.01.The results showed that the control group could effectively improve the thyroid function.After treatment,the thyroid function of the two groups were compared: TSH(49)>0.05,FT3 FT4(49)>0.05 no statistical difference,there was no difference between the treatment group and the control group in the improvement of thyroid function.(3)In the lipid-lowering: the treatment group before and after treatment: TG,CHOL,HDL-C,LDL-C have been significantly improved((49)< 0.01).There was no significant difference in the improvement of TG,CHOL,HDL-C and LDL-C in the control group((49)> 0.05).After treatment,the two groups of blood lipids: the treatment group was significantly better than the control group((49)< 0.01).(4)In terms of clinical symptoms improvement: In the improvement of clinical symptoms,improvement of clinical symptoms before treatment was significantly higher than the control group after treatment,after treatment and the difference before treatment were-9.91 ± 5.81 and-0.33 ± 1.45,the difference was statistically significant,improvement of symptoms in treatment group was significantly better than the control group((49)< 0.01).(5)There was no significant difference between the treatment group and the control group in the improvement of goiter.There was no significant difference between the two groups((49)> 0.05).(6)the curative effect evaluation: to all subjects were evaluated according to the evaluation standard of Hashimoto's hypothyroidism,TCM syndrome curative effect evaluation standard: the treatment group was 19,effective 13,10 cases of the control group,22 cases were effective,curative effect of the treatment group was better than control group(c2= 4.661,(49)=0.031)(7)The patients in the treatment group had no obvious adverse reactions after administration.Conclusions Hashimoto is hypothyroidism of spleen kidney deficiency and phlegm turbidity syndrome,through the use of Shencao Erxian Decoction,and compared with the control group,that can regulate the abnormal thyroid function and blood lipid level,reduce thyroid antibody,alleviate and improve the clinical symptoms,clinical curative effect,safe and reliable,and has broad application prospects;but in the narrow goiter has not yet found a significant effect,which may be in the observation time and the limited number of relevant. |
