Relevant Research Between Cancer-related Fatigue And Hope In Gynecological Cancer Patients

Research purposes:Understand and evaluate the level of cancer-related fatigue and hope in gynecological cancer patients;analysis the related factors of cancer-related fatigue and the level of hope;explore the correlation between cancer-related fatigue and hope of gynecological cancer patients.The study can provide theoretical support and guidance for clinical workers to strengthen the individualized nursing and thus improve the level of hope,relieve cancer-related fatigue,improve the quality of life of gynecologic cancer patients.Research methods:By convenient sampling method,252 gynecological cancer patients,included cervical cancer,endometrial cancer and ovarian cancer,were selected from a third grade class-A hospital of Changchun to conduct questionnaire survey during July2016 to December 2016.The questionnaire was made up of three parts: the general information scale,the Piper Fatigue Scale-Revised(PFS-R)and the Herth Hope index(HHI).Using Epidate to establish date and the operation about data input was carried by two people.SPSS 20.0 software was used for statistical analysis,statistical methods in this study included variance analysis,t-test,Pearson’s correlation analysis,multiple stepwise regression analysis.Research results:1.Cancer-related fatigue was common in gynecological cancer patients.There were 187 patients with moderate fatigue,accounting for a total of 74.2%,the patients with mild fatigue accounted for 9.5%,the patients with severe fatigue accounted for16.3%.The cancer-related fatigue overall average score of gynecologic cancer patients was 4.97 ± 1.24 and belonged to a moderate fatigue level;In four dimensions,the average scores from high to low: physical perception dimension(6.02 ± 1.58),emotional dimension(5.52 ± 1.57),behavior dimension(5.13 ± 1.39),cognitive dimension(3.49±1.09).Among them,physical perception fatigue was severe fatigue,and the other three dimensions were moderate fatigue.2.Single factor analysis showed that the gynecological cancer patients’ age,degree of education,the way of medical payment,payment capacity of medical expenses of the family,clinical stage,course of disease,treatment methods,with or without pain,sleep status was related with cancer-related fatigue.3.Among the gynecological cancer patients invested,there were 177 patients with high level of hope,accounting for a total of 70.2%,the patients with medium hope accounted for 29.8%,no patients appeared with low level of hope.The hope overall average score of gynecologic cancer patients was 37.59±3.64 and at the high level of hope;the scores of three dimensions ranked from high to low: take the positive action(12.81 ± 1.42),maintain close relationship with others(12.62 ± 1.32),positive attitude towards the reality and future(12.17±1.39).4.Single factor analysis showed that the gynecological cancer patients’ age,the way of medical payment,payment capacity of medical expenses of the family,clinical stage,course of disease,number of comorbidities,with or without pain,sleep status was related with hope level.5.Cancer-related fatigue score and the score of each dimension in gynecological cancer patients were significantly negative correlation with the total score of hope and the score of each dimension(P<0.01).6.Multiple stepwise regression analysis showed that six variables,included hope score,with or without pain,sleep good,surgery alone,age,surgery combined with radiotherapy and chemotherapy,entered into the regression equation,and could explain 32.0% of the variance of cancer-related fatigue.(R2=0.320).Research conclusions:1.Cancer-related fatigue were common in the gynecological cancer patients invested,majority of patients had moderate fatigue,and influence factors of cancer-related fatigue were patients’ age,degree of education,the way of medicalpayment,payment capacity of medical expenses of the family,clinical stage,course of disease,treatment methods,with or without pain,sleep status.2.The gynecological cancer patients invested were on the medium and above medium level of hope,and influence factors of hope were patients’ age,the way of medical payment,payment capacity of medical expenses of the family,clinical stage,course of disease,number of comorbidities,with or without pain,sleep status.3.There was a negative correlation between the level of cancer-related fatigue and hope in patients with gynecological cancer.The higher the level of cancer-related fatigue,the lower the level of hope was.4.Multiple stepwise regression analysis showed: hope score,with or without pain,sleep state,surgery alone,age,surgery combined with radiotherapy and chemotherapy were main influence factors of cancer-related fatigue in patients with gynecological cancer.
