Background:Community acquired pneumonia(CAP)has become the leading cause of death among children below five years of age.Each year,about 2 million children die from pneumonia,accounting for about 20%of the total number of deaths worldwide.A study in 2013 showed that the annual morbidity of childhood of CAP in developed and developing countries was as high as 5%and 29%.The virus is the main cause of CAP in children,accounting for 1/3 to 1/2 of CAP in children,especially in infancy and the initial stage of pneumonia,which is also an outstanding feature of aetiology with district difference from adults.The total detection rate of virus decreased with aging and with the highest detection rate in infants.Respiratory syncytial virus(RSV)was ranking first in the general detection of virus,followed with adenovirus(ADV),influenza virus(IFV)and parainfluenza virus(PIV)and so on.CAP in children is more common belonging to "FEI YAN CHURN SOU" disease based on Traditional Chinese Medicine(TCM).The obstruction of lung by wind-cold is a most common and significant kind of syndrome at the initial stage of viral pneumonia,How to scientifically and comprehensively expose the essence and connotation of this syndrome is a big problem to be solved so urgently.Research has indicated that,for the treatment or therapeutics of viral pneumonia among children,there is a great need and so much lack of effective chemical synthesis of drugs,and ribavirin,which is the just recommended by American Academy of Pediatrics declaring with"Maybe you can use".On the other hand,there has been always great advantages of Chinese herbs or other treatments,and it has been definitely verifed by a great amount of clinical cases and experimental lab-findings.Professor Wang Shouchuan,Nanjing University of Chinese Medicine,has benn leading the group engaged in TCM therapy aimed at viral pneumonia during childhood 20 years or so,and established and published the Guidelines for Diagnosis and Treatment of Commom Disease of Pediatrics in TCM,which played a vital role in both prevention and treatment.Metabonomics(Metabolomics),an important branch of systems biology,paying close attention on the nature,quantity,species and overall variation of endogenous metabolites of organism,is a new filed.Metabolomics can effectively avoid the "real world" of predicting or speculating the physiological state or pathology of the previous proteomics or genomics alone using one or more indicators,or one or several gene-blocking pathways,while But can be timely,effective,objective and sensitive mapping of organisms by the external disturbance after the body of genes,proteins,cells or tissues of the "biochemical phenotype" to the overall measure to replace a single test,is very consistent with the basic theory of Chinese medicine,"the overall view,Constant moving view ",is a kind of aerial photography to replace a single jungle search point of view.In the methodology,metabolomics has integrity,dynamic and comprehensive holistic features shared with the basic theory of traditional Chinese medicine.In the philosophical level,there is a match bridge crossing over modern metabolomicsresearch and traditional medicine exposing.Thus,we try to profile viral pneumonia with wind-cold syndrome with the method of metabonomics combined with other biochemical tools,and try to clarify the scientific essence of children's virus pneumonia with wind-cold syndrome.Objective:The whole point of view of children serum and urine of the endogenous differences in metabolic characteristics,of viral pneumonia with syndrome of wind-cold to explain the essence of WC syndrome and to make it clear what is the potential mechanism of metabolic disturbance and adjustment.Methords:Clinical trials of pediatric viral pneumonia with syndrome of wind-cold:In the study of patients suffering from viral pneumonia with syndrome of wind-cold(WC)in 30 cases.while the normal healthy(NH)in 30 cases.Serum and urine samples were collected and both of their metabolites profiles were analyzed by Gas chromatography-mass spectrometry(GC-MS)coupledwith a couple of statistical analyses such as the predealing.analysis platform of XCMS Online and targeted matching platform of NIST database and then pathway-searching platform of MetaboAnalyst in order to explore the essence of viral pneumonia with syndrome of wind-cold.Results:1.Serum metabolomics pattern recognition analysis showed that pneumonia with syndrome of obstruction of wind-cold could be clearly separated from the normal healthy group,several metabolic pathways related organic acids,amino acids,fat were identified.Compared with NH group,the levels of organic acids,amino acids,fat were upregulated,such asL-Alanine,L-Aspartic acid,L-Glutamic acid,Glycine,Serine,L-Threonine,Taurine,L-Lysine,and D-2-Aminobutyric acid,Glycolic acid,Glycerol,1-Monopalmitin,cholesterol,d-Glucose were down in serum with WC group.2.The results of urine metabolomics pattern analysis showed that although there was a tendency of separation between that pneumonia with syndrome of obstruction of wind-cold and normal healthy groups,still could not separated completely with cross and overlap.Further OPLS-DA models were modified and then both groups could be completely detached,signifying a kenspeckle significant metabolic diffrences.The levels of Glycine,L-serine,L-Tryptophan,3-Hydroxybutyric acid,D-Gluconic acid,d-Galactose,Inosine,Uridinewere showed a upward trend,while the levels ofGlycolic acid,Stearic acid,Palmitic acid were showed a downward trend in the urine.Conclusion:1.GC-MS metabolomics platform is a great tool of identifying and initially explaining the metabolic distinction of viral pneumonia with syndrome of wind-cold and normal healthy groups.2.Viral pneumonia among children who were with syndrome of wind-cold mainly marked with Alanine,Glycine,Taurine metabolism,Arachidonic acid metabolism,Palmitic Acid metabolism disorder.
