Effect Of Modified Bazhen Decoction On The Hidden Blood Loss After Total Knee Arthroplasty

ObjectivesTo observe the effect of modified Bazhen Decoction on the hidden blood loss after total knee arthroplasty(TKA)under the guidance of traditional Chinese medicine(TCM)theory.To compare the hemoglobin(HB),hematocrit(HCT)index before and after operation,and to observe the amount of intraoperative bleeding,postoperative drainage and the degree of swelling under the knee,and to calculate the hidden blood loss.To study the clinical effectiveness of modified Bazhen Decoction on reducing the hidden blood loss after TKA.To provide effective treatment and objective clinical basis of TCM for the prevention and treatment of the hidden blood loss after TKA.MethodsSelecting 60 patients from the orthopedics hospital affiliated of Guangzhou University of TCM that accord with the inclusive criteria during February 2016 to February 2017.The patients were divided into observation group and control group by random number table,30 cases in each group.The control group received routine postoperative treatment,and the observation group was treated with modified Bazhen decoction on the basis of treatment of the control group.To observe and record the two groups of patients'changes of HB,HCT index before operation and the 1st,3rd,7th,10th days after operation,and to compare the hidden blood loss between the two groups.All data were analyzed using SPSS 19 software.Results1.There were no statistically significant differences before operation between two groups on HCT,HBC index and the degree of swelling under the knee(P>0.05).2.There were no statistically significant differences between two groups on the amount of intraoperative bleeding,postoperative 48h drainage and dominant blood loss(P>0.05).3.The hidden blood loss after operation of observation group was significantly less than control group so there was statistically significant difference(P<0.05).4.Two groups' HCT and HB index after operation firstly decreased,and then increased,and the changes of these two index between groups were statistically significant differences(P<0.05).There were no statistically significant differences between two groups on the decrease of HCT and HB index on 1st day and 1st to 3rd days after operation(P>0.05).The increments of HCT and HB index on 3rd to 7th days and 7th to 10th days after operation of observation group was significantly higer than control group so there were statistically significant differences(P<0.05).5.There were no statistically significant differences between two groups on HB index on 1st and 3rd days after operation(P>0.05).The HB index on 7th and 10th days after operation of observation group were significantly higer than control group so there were statistically significant differences(P<0.05).6.The degree of swelling under the knee on 3rd,7th and 10th days after operation of observation group were significantly lower than control group so there were statistically significant differences(P<0.05).ConclusionModified Bazhen Decoction can reduce the hidden blood loss after TKA.It can also reduce the degree of swelling under the knee and increase HB index after operation with the prolongation of the time of taking this decoction.
