Clinical Observation Of Modified Gui Pi Decoction Combined With Tranexamic Acid In Treatment On The Hidden Blood Loss After Total Knee Arthroplasty

Objective:To investigate the clinical effect of Modified Gui Pi Decoction combined with tranexamic acid on the hidden blood loss after total knee arthroplasty(TKA),and to provide another way for the treatment of this disease.Methods:70 patients who admitted to be hospitalized in our hospital from January 2019 to December 2019 and who met the inclusion and exclusion criterias.These patients were divided into the control group and the observation group randomly.Control group: tranexamic acid injection l.0 g was dissolved in 5% glucose injection with 100 ml intravenous infusion30 min before TKA.After suturing of the capsule,tranexamic acid l g was diluted in 50 ml normal saline and injected into the joint cavity.Tranexamic acid l g was injected intravenously at 3h and 6h postoperatively.Observation group: on the basis of the control group,from 1 day to 14 days after the operation,the control group was given Modified Guipi Decoction granule,1 dose per day,divided into morning and evening.Hemoglobin(HGB),hematocrit(HCT),intraoperative blood loss and postoperative blood transfusion were recorded in the two groups before and after the operation.The score of the Hospital for special surgery kenee score(HSS)and the score of clinical symptoms of traditional Chinese medicine(TCM)were recorded before surgery,7,14 days and 1 month after surgery.The clinical efficacy of the two groups were compared before and after treatment.All data were analyzed using SPSS 22.0 statistical software.Results:30 and 32 cases completed the study according to the regulations in the observation group and the control group.There was no statistical significance in the general data of gender,age,elevation,weight,and surgical profile of the two groups(P >0.05).1.Laboratory index: comparison between groups: the HGB and HCT levels of the observation group on day 7 and 14 after surgery were higher than those of the control group,and the difference was statistically significant(P <0.05).Intra-group comparison: the HCT level of patients in the two groups showed an increasing trend at 3,7 and 14 days after the operation,and the differences between the groups at 3 days after the operation and 7 days after the operation and 14 days after the operation were statistically significant(P <0.05).2.Blood loss: there was no statistically significant difference in intraoperative blood loss and postoperative blood transfusion between the two groups(P >0.05).The amount of latent blood loss in the observation group was significantly lower than that in the control group,and the difference was statistically significant(P <0.05).3.HSS score: the knee HSS score of the observation group was higher than that of the control group on the 7th day,the 14 th day and the 1st month after the surgery,and the difference was statistically significant(P <0.05).4.TCM syndrome score and efficacy: after treatment,the TCM syndrome score of qi and blood deficiency syndrome in the observation group was higher than that in the control group,with statistically significant differences(P <0.05).The effective rate of treatment in the observation group was 96.67% higher than that in the control group(84.38%),and the difference was statistically significant(P <0.05).5.Safety observation: there were no obvious abnormalities in liver and kidney function and biochemical indicators in the two groups,and no serious adverse reactions or adverse events occurred.In the observation group,2 patients had constipation and 2 patients had dry mouth.The symptoms were significantly improved after the dosage was adjusted,and the drug was not stopped.Conclusion:1.The use of Modified Guipi Decoction combined with tranexamic acid after TKA can increase the serum HGC and HCT levels of patients,reduce the hidden blood loss and the total blood loss after TKA,which is beneficial to the rapid recovery of patients after TKA.2.Modified Gui Pi Decoction combined with tranexamic acid treatment after TKA can improve patients' HSS scores after surgery and help promote early knee function recovery.3.Modified Guipi Decoction combined with tranexamic acid after TKA can reduce the TCM syndrome scores of Qi and Blood Deficiency after TKA,and improve the symptoms of Qi and Blood Deficiency after the operation.
