Objective:To provide advices for the population with high risk of cerebral apoplexy on health management,the study makes a health screen and baseline survey for the people in Yanbian area,founds the high-risk stroke group and creates the 4P medicine database,and analysises the risk factors for stroke.The study aims to explore a suitable health management model for Yanbian high-risk stroke groups and give the suggestions for the stroke prevention and control strategy.In order to promote the national physical quality and health level of population,and finally make a contribution to the universal public health service.Methods:The study is a status quo investigation method in the non experimental research.Choosing the community people in Yanbian area randomly and predicting the risk rating of stroke in the people aged over 30 by physical examination and questionnaire survey.It uses the cardio-cerebrovascular disease risk factors in community population screening table as the research tool.The study uses SPSS17.0 to analyse the datas.Results:1.Demographic characteristics of respondentsIncluding people in this study is 977 totally,female group(520,53.2%)is a little more than male group(457,46.8%);most people belongs to 60~70 years old,and the surveyed people who has a spouse takes 87.2%;main academic degree(398,40.7%)for junior high school,high school/secondary counts(311,31.8%),college degree and the elementary school are following fewer layers,respectively for 147(15.0%)and 121(12.4%).2.Demographic characteristics of population with risk in Stroke2.1 Demographic characteristics of population with high risk in StrokeIn this study,a total of 749 people at high risk of stroke,distribution of men and women are roughly balanced,women are a little more than men.Age distribution in the majority is 60~70 years old people(345)and the least is 81 years old or older(16,2.1%).88.4%of the surveyed people have a spouse.Most people of the high-risk group are from junior middle school and high school,and others are from college and the elementary school.2.2 Demographic characteristics of people with low and middle risk in StrokeIn this study,low-risk and middle-risk people are totally 228,fewer than people at high risk,and accounting for 23.3%of the surveyed population.The women take 58.3%,more than men at 41.7%.Age distribution number order from more to less is 60~70,50~60,under 50 years old,70~80 years old,80 years old and above.In terms of marital status,people who have a spouse are far more than those of no spouse.Academic degree distribution is most by junior high school.3.Stroke risk factor assessment and predictionThe study shows that there are many factors make an influence on stroke.As for the disease project,previous stroke,transient stroke attack,hypertension,diabetes and heart disease are all influencing the risk of stroke previously(P≤0.05);the family history of stroke,coronary heart disease,high blood pressure and diabetes also were statistically significant(P≤0.05);also,age,smoking,drinking and body mass make influencese.4.The logistic analysis for the risk factorsThe logistic analysis is made and it shows that stroke,hypertension,dyslipidemia,diabetes;Family history of stroke family history,family history of hypertension;Lifestyle of smoking,exercise,and body mass index,the influence of other factors have no statistical significance,and the regression model to establish the effective degree was 93.2%.Conclusion:It shows that there are many factors making an influence to the stroke.It is necessary to create a health management system for the high-risk population at stroke.And we should do these below to building the system:Firstly,making an effective using of social medical resources;secondly,construct the stroke unit based on the hospital;thirdly,interacting with community in management;fourthly,making full use of network;finally,strengthen the management system.
