| [Objectives]:1. Aimed to find the demographic characteristics, prevalence of the disease and epidemic situation.2. It can provide theoretical support for the government to formulate relevant laws and regulations.3. Preliminary study on health management model of sanchia hospital.[Methods]:1. The object of study:the study is used the doctor examination from3armour hospital medical center during January1,2008to2012December total of4times in5years as the research object, on the four cross sectional analysis data to study, because each of the participation is not fixed, some data is not complete, the actual, there are895doctors to participate in the examination.2.Research contents:the study was used longitudinal research, doctors material were nearly4times in5years, including:cardiovascular disease: blood pressure, electrocardiogram, blood lipid indexes (low density lipoprotein, triglyceride, total cholesterol); disease of digestive system:liver function index (r-glutamyl transpeptidase, alanine aminotransferase transferase); endocrine disease indexes:blood glucose; abdominal B ultrasound; gynecological B ultrasound; chest radiography; ENT examination:fundus arteriosclerosis.3.The statistical method:Diagnosis was made basing one the general clinical diagnostic criteria. Database was set up with EXCEL, including onset age, gender analysis, engaged in the professional category, distribution of departments and the detection rate of the relationship, study on dynamic change of5years. Physical examination and biochemical indexes related to the definition of the reference of the people’s Republic of China Ministry of health "national clinical laboratory operation rules"2006edition.Interval sample size was estimated with the exact (clopper-pearson) method by PASS11.0. The sample size of single group was at least402people. Using the Pearson χ2analysis in Categorical variables, compared with the Bonferroni correction. Changes of biochemical indexes in5years the doctor used a linear mixed model to evaluate. An unstructured covariance type with repeated linear mixed model (UN), fixed effects into the main effect of gender, age, physical age model, random effect in the constant term, marginal mean compared with, used LSD method, the establishment of health management model as the reference group before2008, P<0.05was statistically significant, with SPSS21.0statistical analysis.[Results]1)Abdominal B ultrasound results of5years before the symptoms of10response percentage were thyroid solid lesions in175(19.5%), hyperplasia of breast164(18.3%), fatty liver166(18.5%), prostatic calcification in114(12.7%), gallbladder polyps in72(8.0%), renal calculi in58(6.5%), hepatic cyst62(6.9%), thyroid cysts48(5.4%), renal cyst in38(4.2%), gallbladder stones in26(2.9%), the abdominal B ultrasound symptom distribution varied.2)5years of gynecological B ultrasound results of response percentage3symptoms were uterus myoma in58(14.9%), cervical cyst in47(12.1%), cervical ectopic columnar epithelium24(6.2%), the highest annual myoma of uterus.3)5years of chest X-ray results in response to3percentage of symptoms followed by X-ray was abnormal in19(2.1%), pulmonary fibrosis in6(0.7%), pulmonary tuberculosis in4(0.4%).4)4other examination results in response to3percentage of symptoms in ocular fundus arteriosclerosis31(3.3%).5)5years of ECG results were statistically significant, the abnormal rate of ECG was lowest in201216.7%(69/412), the abnormal rate of ECG of the2008high of69.2%(313/452),5years total ECG abnormal ratio of52.9%. Physical examination results of5years of low density lipoprotein, triglyceride, total cholesterol were different and had statistical significance. An annual average of2012higher than in2008(P<0.01).6)r-glutamyl transpeptidase5years examination results (P=0.115). Alanine aminotransferase in2008LG alanine aminotransferase marginal maximum is1.39U/L, higher than the20121.36U/L.7)There was significant difference5years examination results of LG glucose (P=0.004), the2008LG glucose marginal is0.69μ mol/L lower than0.70μmol/L,0.71μ mol/L in2010and in2011(P<0.0).[Conclusion]1. The study had a preliminary discussion on the physical examination system in the mode of health management for physicians a SanJia hospital. Abdominal B ultrasound, established gynecological B ultrasound, chest X-ray, electrocardiogram, other diseases, cardiovascular disease, digestive system disease index, index of endocrine system disease index, comprehensive evaluation objective. The positive rate of abdominal B ultrasound distribution showed hyperplasia of mammary glands, thyroid nodules and total positive rate is respectively:19.5%and18.3%, and there is a rising trend. Indicators of cardiovascular disease:5years total ECG abnormality ratio was52.9%, while the low density lipoprotein, triglyceride, total cholesterol, the differences were statistically significant, marginal means were higher in2008than in2012. The digestive system disease index:1g r-glutamyl transpeptidase in5years examination results is no significant(P=0.115).2. Adopt relevant policies, the doctor were followed up and the related items were put into physical examination content, the basic data were sufficient for evaluation after effect of outcome and intervention. |
