| Objectives:Through collecting of the patients with advanced gastric cancer in syndrome differentiation,explored the syndrome elements characteristics,using immunohistochemical method to measure expression of gastric cancer stem cell related gene CD 133 and CD44,and comparing with the adjacent tissues of gastric cancer,to explore the relationship between the syndrome elements characteristics of advanced gastric cancer and expression of gastric cancer stem cell related gene CD 133 and CD44,in order to illuminate the pathogenesis of gastric cancer from two aspects of TCM theory and modern medical science,and provide the basis for relationship between TCM and Western Medicine.Methods:To collect TCM diagnostic information from observation,listening,interrogation,and pulse-taking,then according which the syndrome differentiation was carried out.Main disease location and disease syndrome were found out by statistical analysis.Meanwhile,cancer tissue and adjacent normal tissue(more than 5cm from the tumor)of all the patients were collected.CD 133 and CD44 gene expression were detected by immunohistochemical method(SP method).The relationship between the distribution of gastric cancer syndrome and the expression of CD 133 and CD44 in gastric cancer stem cells was analysed statistically.Results:1.Characteristics of age distribution:60-69 years old dominated in the patients,and male more than female with a ratio of 3:1.2.Characteristics of The syndrome distribution:The main lesion location was the stomach and spleen,then secondly liver and kidney.Major disease syndrome was qi deficiency,then secondly wet,blood stasis,qi stagnation,yin deficiency and heat.3.The expression levels of CD133 and CD44[41.67%(25/60),46.67%(28/60)]were significantly higher than those in adjacent tissues(all of which were 0%),and the difference was statistically significant(P<0.01).The expression of CD 133 gene in gastric cancer tissue was not correlated to age,gender,tumor stage,lymph node metastasis and tumor(P>0.05).While the expression level of CD44 gene was associated with lymph node metastasis(P<0.01),but not associated with age,gender,tumor stage and tumor location(P>0.05).4.Disease location factor "stomach" and "spleen",aslo the syndrome of "Qi" and "wet"and "blood stasis" were correlated to CD133 and CD44(P<0.05),while other syndrome factors were not correlated to CD 133 and CD44(P>0.05).Conclusion:1.The main disease location factor with advanced gastric cancer were the stomach and spleen,followed by liver and kidney.Major disease syndrome was qi deficiency,then secondly wet,blood stasis,qi stagnation,yin deficiency and heat,also seen more.It was suggested the pathogenesis of gastric cancer was the combination of deficiency and excess in terms of TCM theory.The deficiency of the spleen and stomach deficiency mainly,while it was mainly composed of dampness,blood stasis,stagnation of Qi,excess heat and phlegmin in the aspect of excess.2.The positive expression rates of CD 133 and CD44 in gastric cancer tissues were significantly higher than those in adjacent tissues,and the expression of CD44 gene was correlated with lymph node metastasis.It can be seen that the expression level of CD 133 and CD44 has important significance in the diagnosis of advanced gastric cancer,It is of significance to predict the risk of metastasis of gastric cancer by CD44 gene expression level.3.Disease location factor "stomach" and "spleen",aslo the syndrome of "Qi" and "wet"and "blood stasis" were correlated to CD133 and CD44 while other syndrome factors were not correlated to CD133 and CD44.That is the expression of CD133 and CD44 gene in gastric cancer tissues with different syndrome elements is inconsistent.It has a positive significance to explain the microscopic mechanism of gastric cancer syndrome,and to better explain the pathogenesis of gastric cancer,western medicine and the relationship between the two. |
