The Research Of Professor Wang Canhui In Treating Chronic Atrophic Gastritis Based On Data Mining

Objective:Based on the several years clinical practice of Professor Wang Canhui,a famous TCM doctor,the experience of Professor Wang on chronic atrophic gastritis was studied with data mining technology,in order to inherit and develop the clinical experience of famous TCM doctors.Method:The medical records of chronic atrophic gastritis diagnosed and treated by Professor Wang from March 2012 to November 2016 were collected and screened strictly in accordance with the inclusive and exclusive criteria.The medical records after screening were pretreated and a database was established for the clinical materials of the 158 cases that met the requirements.By applying frequency analysis,association rules,cluster analysis and other methods,contents of symptoms,syndromes,prescription and Chinese herbs in the medical records were dug out and the results were analyzed and summarized combining with the clinical experience of Professor Wang.Results:Among the 76 cases of men with 235 times of visit and 82 cases of women with 183 times of visit in the whole 418 times,the average age was 54.20 years old.The main symptoms possessing higher frequency included gastric distension and fullness,stomach dull pain and epigastric upset,accounting for 58.37%,22.97%and 12.20%in all visit times respectively.The accompanied symptoms possessing higher frequency included dry mouth,poor appetite,blenching,bitter mouth,loose stool,constipation,mental fatigue and lack of strength and poor sleep,accounting for 42.34%?27.03%?26.08%?19.14%?15.79%?10.05%?9.81%?8.13%in all visit times respectively.The main combination of tongue pictures included pale tongue with thin yellow coating,pale tongue with thin white coating and dark red tongue with thin white coating,the frequency of which were 21.77%?19.38%?10.77%respectively.The main pulse manifestation included thready pulse,wiry pulse,thread and wiry pulse,the frequency of which were 90.43%?43.06%?33.49%respectively.The main syndrome patterns included spleen and stomach deficiency,spleen and stomach dampness-heat,blood stasis of stomach collateral and qi stagnation of liver and stomach,with the respective frequency of 87.80%?33.73%?32.78%?32.30%.Among the 39 Chinese herbs with the application frequency of more than 5%,drugs with the frequency of more than 65%included Baizhu,Taizishen,Ezhu,Zhike,Xianhecao,Jineijin,Baihuasheshecao and Fuling,which were also the key drugs of medicine association.Drugs with the frequency of more than 7%were mainly belonging to spleen-tonifying and qi-boosting drugs,heat-clearing and dampness-removing drugs,blood-activating and stasis removing drugs,digestion-promoting and accumulation-removing drugs and qi-rectifying and the center harmonizing drugs,the cumulative frequency of which could reach 65.96%.The main prescriptions with the application frequency of more than 50%included Zhizhu Pill and Sijunzi decoction.2 prescriptions and 4 groups of drug pairs were received by cluster analysis.Conclusion:The study found that spleen and stomach qi deficiency was the basic pathogenesis of this disease and the accompanied syndrome included dampness-heat,heat accumulation,yin deficiency and disharmony of qi and blood.The main treatment is based on spleen-tonifying and qi-boosting drugs,heat-clearing and dampness-removing drugs,blood-activating and stasis removing drugs,digestion-promoting and accumulation-removing drugs as well as qi-rectifying and the center harmonizing drugs,with the basic treatment method of boosting qi and clearing heat,rectifying qi and harmonizing blood.Great attachments to the combination of disease and syndrome and the usage of mild medication in a flexible way are cinsdered as the treatment characteristics,with equal emphasis being stresed on tonification and elimination,nourishment and dampness removal,astringing yin and dispaelling stasis.
