Study On Principles Of Differentiation Of Thoracic Obstruction

Objective:Nowadays,the study results of syndrome differentiation of thoracic obstruction are abundant.Although the classification of diagnosis standard of thoracic obstruction has been published(appendix I),the six patterns involved cannot include the complex patterns occurring in clinic.Many doctors still treat the disease on their clinical experiences.The study of Thoracic Obstruction TCM syndrome type has not reached a unified standard yet.The clinic treatments are mainly based on syndrome differentiation.There still remains a big space for the further research of the standardization of Thoracic Obstruction in modern Chinese medicine.Based on the guidance of Professor Wu s new system of five viscera syndrome differentiation system,this study mainly discussed the principles of differentiation of Thoracic Obstruction through the study of disease 's etiology and pathogenesis,syndrome differentiation,syndrome,combining Thoracic Obstruction patients' basic information,and did statistics analysis using epidemiology research methods.Significance:Using the five viscera syndrome differentiation system into the study of mathematical model syndrome differentiation makes the disease and syndrome differentiation of Thoracic Obstruction more flexible,more accurate,easier to master,and more in line with the clinical combination of disease and syndrome differentiation of ideas,which plays an important guiding role in improving the level of clinical diagnosis and treatment.According to the basic principles of "differentiating patterns from syndrome" and "combining pattern differentiation with disease differentiation",this study first used the five viscera syndrome differentiation system into the study of Thoracic Obstruction research by literature review and the epidemiological investigation method,using the TCM diagnosis way of thinking of disease differentiation comes first,pattern differentiation follows,and constructed a new thinking model of Thoracic Obstruction differentiation.Methods:Through the collection and reading of a large number of Chinese literatures,this research made a systematic summary and analysis of literatures in the past dynasties related to chest stuffiness and pains.Firstly,this paper had a research on history of the conception about disease of chest stuffiness and pains.Secondly,the paper sorted out ancient literatures of etiology,pathogenesis,nature of disease and location of disease related to chest stuffiness and pains.The paper discussed on the new cognition of chest stuffiness and pains by combining modern theory with collateral disease.There were so many opinions about differentiation pattern and differentiation methods in history,but modern differential diagnosis of TCM have different ideas which are not standardized.This paper combined concise specification of five Zang-organs on pattern differentiation with mathematical models to make complex process of differentiation become easy and make the information of chest stuffiness have different levels of change.Through the investigation method of retrospective epidemiology,438 cases of chest stuffiness cases were collected after making symptoms normative,using cluster analysis and principal component analysis and statistical analysis method by SPSS statistical software.Disease location and disease nature distribution of chest stuffiness were analyzed.The" information table of disease pattern diagnosis of chest stuffiness " was constructed to standardize the disease location,syndrome elements of chest stuffiness and summarize the basic combing principles of basic and compound patterns of chest stuffiness.Apply fuzzy mathematical principle to construct fuzzy membership function and set up the mathematical model for differentiation.Apply correlation analysis to calculate its confidence and take differentiation model test at last.Result:The paper put forward the basic pattern of " phlegm and stasis stuck in heart"by sorting out ancient literatures and the results of survey of 438 cases of chest stuffiness cases,and put forward compound patterns combing disease location and disease nature of chest stuffinessto promote the further modification of chest stuffiness's modern TCM differentiation.The paper set up the mathematical model of chest stuffiness and have preliminary discussions on the differentiation model.
