ObjectiveTo analyze the association between the serum prealbumin(PA)level and neonatal behavioral neurological assessment in neonates with hypoxic-ischemic encephalopathy(HIE).MethodsA total of 24 neonates including 11 males and 13 females were selected from HIE newborns admitted to Jinan Central Hospital during Jan.2014 to Dec.2015,the neonates were divided into 3 groups according to the HIE diagnostic criteria and clinical grading in 2005,mild(n=11),moderate(n = 9),and severe(n= 4),and another 20 healthy newborns served as controls.Blood samples and NBNA scores were collected from all newborns on day 1,3,and 10 after birth,to detect the serum PA values.Result1.The neonates in each group were no significant difference in gender, delivery way, birth weight and gestational age(P>0.05).2.PA level of the neonates with hypoxic-ischemic encephalopathy were lower than the control group. The Bonferroni's test indicated that on dayl and 3,there existed such relationship among PA level of 4 groups:PA(severe)<PA(moderate)<PA(mild)<PA(control),(P<0.01)3.PA level of the neonates with hypoxic-ischemic encephalopathy were lowest in day3,PA value of each neonates in dayl was higher than that in day3,which conform to the half-life period of PA,1.9day.4.The Bonferroni's test indicated that for all day1,day3 and day 10,there existed such relationship between NBNA scores of 4 groups:NBNA(severe)<NBNA(moderate)<NBNA(mild)<NBNA(control)(P<0.01).5.After treat, the PA level and NBNA score of the neonates with hypoxic-ischemic encephalopathy in day1 were lower than these in day10(P<0.01).6.The Pearson's Correlation Coefficient test showed significant positive linear correlation between PA level and NBNA scores of each HIE group, for dayl(r=0.554,P<0.01),after treat,day10(r=0.344,P<0.01).Conclusion1.As the clinical grading became higher, the serum PA values were found lower. It suggested that PA level could reflect the severity of HIE,which influenced the nutrition state of HIE neonate;2.As the clinical grading became higher, the NBNA scores were found lower. It suggested that NBNA scores could reflect the severity of HIE.3.There was significant positive linear correlation between PA level and NBNA scores. It suggested that PA level and NBNA scores could reflect the severity of HIE together. The serum PA level combined with NBNA scoring could be used as the indicator of early diagnosis and severity of neonatal HIE,to assess the prognosis of HIE neonates.4.Along with the recovery of disease, the serum PA values and NBNA scores were gradually increased. It suggested that the serum PA combined with NBNA scoring could monitor the recovery condition and guide therapy.
