The Clinical Observation Of Treating As Attributed To Pattern Of Yang Deficiency With Cold Dampness By Methd Of Warming The Kindney And Invigorating The Governor Vessel

Objective:Due to the chronic pain,inconvenience life persists over patients suffered from Ankylosing spondylitis(AS);however,the efficacy of modern medicine for its clinical symptoms is limited.Modified Yanghe Decoction(MYD)is refined from the experience of treating AS by professor WANG Yue.We objectively appraised the treating result of MYD on pattern of Yang deficiency with cold-dampness for AS patients by systematic clinical study,especially for the influence on aspects of clinical symptoms,signs,and indexes of laboratory,and evaluate its efficacy and the improvement on life quality to provide objective evidence for clinical application.Method:According to the control and distribution by diffemt level principle of clinical trial,60 AS patients on pattern of Yang deficiency with cold-dampness were divided into three groups as Traditional Chinese Medicine(TCM)group,western medicine group,and combination of TCM and western medicine group with 20 cases in each,respectively.MYD was administered for TCM group,celecoxib capsules and sulfasalazine enteric coated tablets were administered for western medicine group,and MYD,celecoxib capsules,and sulfasalazine enteric coated tablets were all administered for combination medicine group.Each course of treatment was 4 weeks,and the efficacy was evaluated after 2 courses of treatment by the standard scores of TCM syndrome,and indexes of Ankylosing Spondylitis Quality of life(ASQoL)and Assessment in Ankylosing Spondylitis(ASAS20).The change level of disease was evaluated by the scores of BSDAI and Bath Ankylosing Spondylitis Funetional Index(BASFI),and indexes of ESR,CRP in serum.Evaluation of quality of life of patients before and after treatment with ASQoL score.Results:After clinical observation over 8 weeks for 60 patients,the efficacy generally lied on the decrease of disease activity,clinical symptoms,signs,TCM syndroms,indexes of labotorary,and quality of life(P<0.05).The clinical symptoms on level of pain,indicators of inflammation,BASFI improved significantly in the combination medicine group(P<0.05).The ESR and CRP improved significantly(P<0.05)in all 3 groups,while the score of ASQoL was higher in the combination medicine group than that in TCM group and western group.Amoung all 3 groups,only 2 cases of adverse incidents happened in western medicine group,both of which were slight stomach discomfort and diarrhea;however,patients agreed to continue the medication after communication without special treatment and no other significant side effect ever happened on the other cases.Conclusion:The combination of MYD and western medicine on AS based on the method of warming Kidney to invigorate the governor vessel and warming the collateral to dissipate cold,leading into an affirmative clinical efficacy on improving not only the symptoms and signs,but also the disease activity and life quality without abnormalities in safety observation,will have widely future application.
