Background: Hearing impairment not only affects the physical and mental health of the disabled,but also causes the social problems such as the burden of family and society.The use of hearing aids can help them improve their listening and communication skills,reduce their barriers to social life and improve their lives.In this paper,we study the changes of quality of life,quality of care and the attitude to disability of aural disabled persons before and after wearing hearing aids.Objective: To understand the quality of care,attitude to disability and quality of life of aural disabled persons.To evaluate the effect of hearing aids on them by three aspects: quality of care,attitude towards disability and quality of life and analysis the relationship between the before and after wearing hearing aids.In order to provide suggestions for the provision of care,the improvement of the attitude and the improvement of the quality of life of aural disabled persons.Methods: A survey was conducted on 198 aural disabled persons in Hangzhou city by using the WHO quality of care scale,the attitude to disability scale and the quality of life scale.The relationship among the quality of care,attitude to disability and the quality of life of them was analyzed by single factor and multiple factor analysis.Also through a return visit to compare the quality of care,attitude to disability and quality of life before and after hearing aids.Results:(1)The main causes of hearing disability in Hangzhou included drug-induced,otitis media,nerve,tympanic membrane perforation,sudden and hereditary;(2)66.2% of people believed that their own quality of life in general;the average score of quality of life was 64.17±9.30 and the lowest score is physiological domain.The correlation between quality of life and psychological domain was the largest(r=0.895);(3)The average score of quality of care was 54.07±8.68 and attitude to disability was 50.46±7.48;(4)Quality of care,attitude to disability and quality of life were positively correlated(r=0.491,r=0.439);the results of path analysis showed that the main influencing factors of the quality of life were caring environment,tolerance,the accessibility of care and care giving;(5)A total of 195 people with hearing disability accepted a return visit,the rate was 98.5%;in the past 1 years,there were 44 person did not wear hearing aids,and151 wear hearing aids,of the 151 person there were 59 people wear occasionally,92 people often wear;(6)The quality of life improved after wearing hearing aids(t=2.041,P=0.043)and the main change were physiological and psychological domains;the attitudes to disability also improved(t=5.237,P<0.001)and all four domains had improved;there was no statistical difference in the quality of care for aural disabled people before and after the hearing aids;people who wear hearing aids often improved their quality of life and attitude to disability better than occasionally;(7)The attitude to disability had a great influence on the quality of life after wearing the hearing aids(standardized partial regression coefficient =0.357).Conclusion:(1)The quality of care,attitude to disability and quality of life of aural disabled people in Hangzhou need to be improved;(3)Quality of care,attitude to disability and quality of life are positively correlated.In order to improve the quality of life of hearing impaired people,it is necessary to strengthen their health management,especially rehabilitation services,and give support to them.At the same time,we should pay more attention to their mental health,help them to establish a positive attitude to life and face the difficulties caused by disability positively.(2)The use of hearing aids has a positive impact on the quality of life and attitude to disability,and wear often the better.Therefore,for the people who do not wear hearing aids,we need meet their health needs.Do our best to help them wear hearing aids,and perfect the hearing aid follow-up services,then help them adapt tohearing aids,so as to improve the effects of hearing aid properly.Through these measures their physical health and their quality of life can be improved.
