| ObjectiveXianFangHuoMingYin is a commom recipes in the treatment of pyocutaneous with Yang-syndrome,which is effective and widely used in treating ance.But there is no evidence to prove its effectiveness and safety.This research affords an evidence-based support for XianFangHuoMingYin in the treatment of acne through the systematic review and clinical observation.Methods1.Meta analysis:We retrieved Chinese and foreign electronic database through computer.The retrieval time was before December 31,2016.The experimental group was treated with XianFangHuoMingYin decoction as the main treatment,white The control group was treated with western medicine or proprietary Chinese medicine as the main treatment.We screened related literatures in strict accordance with the inclusion criteria and exclusion criteria,then extracted the data and evaluated the quality for included literatures.In this meta analysis,efficienty,incidence of adverse reactions and lesion score were evaluated by the use of Revman 5.3 software.2.Clinical observation:80 patients with phlegm and blood stasis syndrome were randomly divided into treatment group and control group.40 cases in the experimental group was treated with the XianFangHuoMingYin decoction,white 40 cases in the control group was treated with Minocycline Hydrochloride Capsules,and both of them were combined with fire needle,four weeks per treatment,a total of two courses of treatment.after that we evaluated its TCM symptom score,lesion score,comprehensive effectiveness and incidence of adverse reactions.Results1.Meta analysis:A total of 13 literatures were included,including 711 cases for the experimental group and 682 cases for the control group.Meta analysis results show that:EfficientyXianFangHuoMingYin group is superior to western medicine group OR=3.03,95%CI[1.90,4.83],P<0.00001)and proprietary Chinese medicine group(OR=2.22,95%CI[1.10,4.45],P=0.03<0.05)in the efficienty of treatment ance,and Combination of the Western Medicine and Traditional Chinese Medicine group is superior to western medicine group(OR=3.34,95%CI[2.23,4.99],P<0.00001).But compared with Retinoids Acid,the efficient is almost the same(OR=1.11,95%CI[0.51,2.43],P=0.79>0.05).Incidence of adverse reactionsThe incidence of adverse reaction of XianFangHuoMingYin group is lower than western medicine group(OR=0.33,95%CI[0.20,0.56],P<0.0001),while compared with proprietary Chinese medicine group,the incidence of adverse reaction is almost the same(OR=3.09,95%CI[0.31,30.40],P=0.33).lesion scoreOnly two included literatures has evaluated the lesion score after treatment,and both of Intervention for control group is proprietary Chinese medicine.The results show that XianFangHuoMingYin group is superior to proprietary Chinese medicine group in the improvement of lesion after treatment.2.Clinical observation:In this 80 cases with phlegm and blood stasis syndrome,the experimental group lost 3 cases,the control group lost 2 cases,and there were 75 cases could be evaluated finally.baseline comparisonUpon examination,there was no statistic difference in gender,age,course of disease,lesion score before treatment,TCM symptom score before treatment between this two groups.lesion scoreThere were improvement of lesion for both of two groups after treatment of 4 weeks,P<0.05,and the difference was statistically significant.There was no difference in lesion score after treatment of 4 weeks between experimental group and control group,P=0.480(P>O.05),and difference value between pre and after 4 weeks treatment was no difference either,P=0.060(P>0.05).There was significant difference in lesion score after treatment of 8 weeks between experimental group and control group,P=0.002(P<0.05),and so did the difference value between pre and after 8 weeks treatment,P=0.010(P<0.05).All this proofs that both of the treatment plans are effective.After treatment of 4 weeks,there was no difference in lesion score between two groups,while with the prolongation of treatment,the experimental group was superior to the control group in the improvement of lesion after treatment of 8 weeks.TCM symptom scoreThere were improvement of TCM symptom for both of two groups after treatment of 4 weeks with statistical significance(P<0.05).Between the two groups,there was significant difference in TCM symptom score after treatment of 4 weeks,P=0.002(P<0.05),and so did the difference value between pre and after 4 weeks treatment.The TCM symptom score was significant difference after treatment of 8 weeks between the two groups,P=0.000,and so did the difference value between pre and after 8 weeks treatment.By comparison,we can know that both of the treatment plans can inprove the TCM symptom,while the experimental group was superior to the control group.Total effectivenessThe total effective rates of experimental group was 97.30%,while the control group was 84.2%,and the difference was statistically significant,P=0.006(P<0.05).It proofs that the experimental group was superior to the control group in total effectiveness.Conclusion1.Meta analysis:For the efficienty,XianFangHuoMingYin is superior to western medicine and proprietary Chinese medicine,while compared with Retinoids Acid,the efficient is almost the same.Combination of the Western Medicine and Traditional Chinese Medicine is superior to western medicine group only.For the incidence of adverse reactions,experimental group is lower than control group,while compared with proprietary Chinese medicine,the incidence of adverse reactions is almost the same.For improvement of lesion,XianFangHuoMingYin is superior to proprietary Chinese medicine.However,in view of the low number and poor quality of included literatures,accuracy and reliability of the results may be affected.2.Clinical observation:Compared TCM symptom score,lesion score,comprehensive effectiveness and incidence of adverse reactions,we comfirm the effectiveness of the two treatment plans,and experimental group is superior to control group in TCM symptom score,lesion score,and comprehensive effectiveness.While the incidence of adverse reactions is almost the same.XianFangHuoMingYin is a wonderful recipe for treating ance,which is worth to be spread. |
