| Objective:This essay aimed to further improve the discharged patient follow-up system by exploring and analyzing its establishment and implementation in one tertiary hospital.Methods:With reference to both domestic and foreign medical institutions, one tertiary hospital established its patient follow-up system in Feburary 2012, hereafter, they continued to improve it. Documents from its discharged patient database were retrieved; patients'data from December 2012 through December 2015 before and after the implementation of the follow-up system were randomly selectd. With literature review, consulting experts and questionnaire, the overall degree of satisfaction, patient teaching, patient's knowledge about common diseases, follow-up situation, depression change, life quality after discharge, and the reliance on continued care and nursing, etc. were collected and contrasted. The implementation effect, existing problems and corresponding solutions were discussed.Results:(1) the control group degree of satisfaction was 69.84%, the follow-up group satisfaction was 95.07%, and the difference is statistically significant (p< 0.05). (2) in the control hospital, the ratio of nursing instruction for patients with wound, rehabilitation training, dieting, medication and informing the patient to hospital for reexamination were 63.76%,69.58%,70.72%,70.46% and 70.72% respectively, in the follow-up group, the indicators were 90.40%,92.30%,88.32%,94.46% and 96.11%, compared with control group, the follow-up group index proportion is higher, the difference is statistically significant (p< 0.05). (3) the control rate of common clinical symptom, treatment, adverse reaction and prevention knowledge is 48.15%, 45.06%,44.00% and 45.33% respectively; the follow-up group index is 62.98%, 65.31%,62.20% and 65.31%; compared with control group, the follow-up group index proportion is higher, the difference is statistically significant (p< 0.05). (4) 61.99%of control group hospital positively contacted its patients while in the follow-up group hospital 93.25% recalled its patients. Compared with control group hospital, the ratio of voluntary call from follow-up group hospital to patients increased significantly, which is statistically significant (p< 0.05). (5)control of SAS and SDS score (51.5-5.4), respectively (50.9-4.1) points, follow-up group of SAS and SDS score (43.3-5.8), respectively (41.2-4.6) points, follow-up group compared with control group SAS and SDS scores are low, the difference is statistically significant (p < 0.05). (6) The body pain, physical function, mental health, social function, overall health in the control group are scored as (6.51-1.23), (6.48-1.35), (6.47-1.30), (6.49-1.27) and (6.31-1.28) respectively, the follow-up group index is scored as (8.29-1.33), (8.38-1.29), (8.29-1.22), (8.29-1.40) and (8.28-1.35) respectively, compared with control group the index score in the follow-up group is higher, statistical difference (p< 0.05). (7)Control group with treatment and nursing, timely taken medication, calm, scientific diet, proper exercise proportion were 69.31%, 70.90%,77.51%,75.49%, and70.46%, the follow-up group index were 88.49%, 93.08%,88.93%,87.72% and 83.56%, compared with control group the index proportion in the follow-up group is higher, statistically significant difference (p< 0.05).Conclusion:(1) the establishment and perfection of the follow-up system helps to promote patient's satisfaction for medical service, improve clinical service quality and hospital management. (2) the establishment and perfection of the system can improve the patients'knowledge of the disease, reduce anxiety and depression, and enhance doctor-patient relationship and mutual trust. (3) the establishment and perfection of the system is also helps to improve patients's life quality and rliance on follow-up treatment. |
