Clinical Study Of Jiangtang Baoshen Decoction Treatment Of Diabetic Nephropathy Stage ?-?:Urine FN And Col-? Change

Objective To observe the therapeutic effect of Jiangtang Baoshen Decoction in the treatment of ?-? diabetic nephropathy, analysis the relationship between urine fibronectin(FN), Type IV collagen(Col-?),24h urinary protein(24hPro), renal function and clinical symptoms, explore the possible mechanism of the marker protein in the development of Diabetic Nephropathy (DN), to develop a new field and provide the new way for the research of Chinese medicine treatment of diabetic nephropathy.Methods 60 cases were received a random arrangement and assigned to "Jiangtang Baoshen Decoction" treatment group (30 cases) and control group (30 cases). The treatment course is 30 days. Chinese medicine syndromes and the urine fibronectin(FN), Type IV collagen(Col-IV), quantitative test of 24h urinary protein(24hPro), serum creatinine(Scr),blood urea nitrogen(BUN),albumin(ALB)and fasting plasma glucose(FPG) were detected before and after treatment, evaluate the clinical effects.Results 1.In the aspects of improving the total clinical efficacy,after the treatment,10 cases were markedly effective,8 cases were effective and 5 cases were invalid in the treatment group (23 cases), the total effective rate was 78.3%, in the control group,8 cases were markedly effective,6 caseswere effective and 4 cases were invalid in 20 cases, the total effective rate was 70%., clinical symptoms between the two groups was statistically significant difference (P< 0.05)The efficacy in the treatment group was significantly better than the control group.2. Comparing the urine protein and urine FN, Col-IV,the index of FN and Col-? has no significant difference before and after treatment in the control group(P>0.05),but there was statistical significance in 24 hour urinary protein quantitative difference before and after treatment(P<0.05), the improvement of 24 hours urine protein and urine FN, Col-IV in the treatment group was significantly better than the control group(P<0.05).3. In the improvement of renal function, the decrease of Scr and BUN was not significant in the control group (P>0.05),but the treatment group can significantly reduce the index of Scr(P<0.05)and the curative effect is better than that in the control group(P<0.05). For the survey of BUN,there is no significant difference before and after the treatment in the treatment group(P>0.05).Conclusion 1.Traditional Chinese medicine compound Jiangtang Baoshen is strict in formula and closely linked to the pathogenesis, the total effective rate was 80.0%,better than the control group, and there were not obvious adverse reaction, were safe and reliable curative effect in the treatment.2. Possible mechanism of traditional Chinese medicine compound Jiangtang Baoshen Decoction in the treatment of diabetic nephropathy listed in the following:this decoction may reduce excessive accumulation of ECM, thereby reduce urine FN and Col-IV, improve blood flow dynamics in diabetes nephropathy patients, delay renal tubular atrophy and interstitial fibrosis to a certain extent.
