Objective: To investigate the relationship between serum 25-hydroxyvitamin D levels and bronchial asthma in children,and analyz the relationship between vitamin D intervention and asthma control.Methods:1 The study group was 152 cases of child with bronchial asthma in the department of paediatrics of our hospital from January 2015 to June 2016,with 90 were males and 62 females,aged from 5 years to 11 years(mean age was 6.9±2.79 years).45 healthy children were used as control group,with 28 were males and 19 females,aged from 5 years to 12 years(mean age was 7.1±2.22 years).The serum levels of 25(OH)D in the two groups were detected by High performance liquid chromatography tandem mass spectrometry,TIgE by electrochemiluminescence,and lung function indexes(FEV1,FVC,FEV1/FVC,MEF25,MEF50,MEF75)were measured.The relationship between serum levels of 25(OH)D and IgE and lung function in the two groups were analyzed.2 In accordance with the guidelines for the diagnosis and treatment of bronchial asthma in children in China,the children with asthma was divided into mild continued group(n=67),moderate continued group(n=53)and severe continued group(n=32)according to the severity of asthma.Serum levels of 25(OH)D and TIgE were observed in mild continued group,moderate continued group and severe continued group.The correlation between the serum levels of 25(OH)D and IgE and the severity of asthma were analyzed in the three groups.3 The mild continued group was randomly divided into the intervention-mild continued group(n=34)and the non-intervention-mild continued group(n=33).The moderate continued group was randomly divided into the intervention-moderate continued group(n=27)and the non-interventionmoderate continued group(n=26).The severe continued group was randomly divided into intervention-severe continued group(n=16)and non-interventionsevere continued group(n=16).All patients were given basic treatment,and the intervention-mild continued group,intervention-moderate continued group and intervention-severe continued group were treated with 800 IU vitamin D every day on the base of basic treatment.The course of treatment was 8 weeks in all the groups.After the treatment,asthma control(complete control,partial control and non-control)of all the patients receiving treatment were observed and compared by children asthma control test.The relationship between vitamin D intervention and asthma control was analyzed.Results:1 The serum 25(OH)D levels in the study group(16.6±5.14 ng/ml)was significantly lower than those in the control group(42.7±6.03 ng/ml)(P<0.01).The serum TIgE levels in the study group(523.9±70.67 ng/ml)was significantly higher than those in the control group(229.4±32.31 ng/ml)(P<0.01).2 Pearson correlation analysis results showed that serum 25(OH)D level was negatively correlated with the serum TIgE level(P<0.01)in the study group.But in the control group,there was no correlation between serum 25(OH)D level and serum IgE level(P>0.05).3 Pearson correlation analysis results showed that the lung function indexes of MEF25,MEF50 and MEF75 were positively correlated with serum 25(OH)D level(P<0.05)and were negatively correlated with serum IgE level(P<0.05)in the study group.But there were no correlation between the pulmonary function indexes and the serum levels of 25(OH)D and IgE(P>0.05).4 Serum 25(OH)D le vels in mild continued group(18.7±3.2 ng/ml),moderate continued group(15.9±4.66 ng/ml)and severe continued group(13.3±4.17 ng/ml)were decreased,and the serum TIgE levels in mild continued group(497.1±72.74 ng/ml),moderate continued group(546.3± 70.39 ng/ml)and severe continued group(647.6±76.42 ng/ml)were increased,gradually.The two groups were compared respectively,the differences of serum levels of 25(OH)D and TIgE were statistically significant(P<0.05).5 Pearson correlation analysis results showed that the severity of asthma was negatively correlated with Serum 25(OH)D level(P<0.05),and positively correlated with serum TIgE level(P<0.05).6 The control rate of intervention-mild continued group(97.1%)was significantly higher than that of non-intervention-mild continued group(78.8%)(P=0.021).Though the control rate of intervention-moderate continued group(81.5%)and intervention-severe continued group(75.0%)were higher than that of non-intervention-moderate continued group(65.4%)(P=0.184)and non-intervention-severe continued group(56.3%)(P=0.264),there was no significant difference between the two groups.Conclusion:1 The level of serum 25(OH)D in children with bronchial asthma is lower than in the healthy children;Serum 25(OH)D levels have a significant relationship with small airway dysfunction in children with asthma.Lack of vitamin D in children with bronchial asthma may be one of the important causes of the asthma attack.2 The serum 25(OH)D levels in children with asthma was negatively correlated with the severity of asthma,and the decrease range increased with the severity of asthma.3 For children with asthma,vitamin D supplementation can improve the clinical symptoms and be advantageous to the asthma control.
