Objective To investigate the similarities and differences expression of keratin 16(K16),interleukin 1 beta(IL-1?),interleukin 17(IL-17)in different TCM(Traditional Chinese Medicine)Syndromes patients of psoriasis and chronic eczema to provide an objective theoretical basis for the identification of different TCM Syndromes patients of psoriasis and a clinical identification between plaque psoriasis and chronic eczema.We also hope that by observing the relationship between epidermal differentiation and immune mechanism among the whole course of psoriasis to observe the law between them.Methods Immunohistochemical method was used to observe the difference expression of these markers in different patients.Results 1 CL(Center lesions)1.1 K16 in the PP(patients of psoriasis)of BHS(blood heat syndrome),BDS(blood dry syndrome),BSS(blood stasis syndrome)has no significant difference in the CL,all expressed in the epidermal layer;All BHS and one of two BDS patients experssed IL-1? in the upper epidermis,the other BDS patient and all BSS patients have no expression in the CL;Five cases of patients with BHS expressed IL-17 in the upper epidermis?dermal layer1 especial the papillary layer and the dilated vessels region,one cases was failed,The patients of BDS expressed in the same part but the number lesser,The patients of BSS in epidermis were rare,and there were more positive cells in the superficial dermis in the CL.1.2 The K16 expression in the whole epidermis was not significant at different stages of the disease in the CL;7 days-1 months CL expressed IL-1? in the upper epidermis when no expression was found in patients with CL over two months;7 days-1 months CL expressed IL-17 in the upper epidermis?dermal layer1 especial the papillary layer and the dilated vessels region when the CL over two months epidermis expressed rare,but more positive cells in the superficial dermis.1.3 K16 expressed in the whole epidermis of ECP(Patients of chronic eczema)in the CL,and no expression of IL-1? was observed;The epidermis of IL-17 was rare and there were more positive cells in the superficial dermis.2 EL(Edge of skin lesions)2.1 K16 expressed in the basal layer on the 2-3 layer of BHS in the EL,expressed 2-3 on the basal layer is deep and weak in 4-5 layer of BHS,in addition to the stratum corneum outside the full-thickness expression of the epidermis of BSS;With the longer course of skin lesions,the level of expression in the epidermis of the whole layer is getting closer to the stratum corneum in the EL;In the EL of ECP expressed in the full-thickness of the epidermis in addition to the stratum corneum. 2.2 The PP of BHS(except one),BDS,BSS,the ECP,the NS had no expression in the EL of IL-1?.2.3 The PP of BHS,BDS,BSS,the ECP,the NS had found a small number of positive cells in the superficial dermis in the EL of IL-17.3 NS(Normal skin)K16 expressed in the Basement upper 2-3 layer of NS(Normal skin)in the CL and no expression of IL-1 was observed;Several IL-17 positive cells were seen in the superficial dermis.Conclusions K16 in(course)expression difference in EL of PP and ECP;IL-1?and IL-17 had significant differences in the CL of PP in different TCM syndrome(course)and ECP,therefore: 1.The difference between them can provide an objective theoretical basis for the different TCM syndrome;2.K and IL show a process of dynamic change in expression of these lesions in different locations,provide a direction for further research in the pathogenesis of the initiating factor;3.The expression of IL in psoriatic plaque stage and chronic eczema patients,especially IL-17,which provides a possible direction for differential diagnosis of two diseases.
