Study On Standardization Of TCM Syndromes Of Psoriasis Vulgaris In Yunnan Province Based On Latent Category Analysis

Objective To classify the TCM syndrome elements of psoriasis vulgaris and explore the syndrome distribution law of psoriasis vulgaris patients in Yunnan.Methods On the basis of the existing literature research,Delphi method was used to carry out expert questionnaire survey,and formulate the TCM clinical four diagnostic information questionnaire for psoriasis vulgaris.Through the clinical questionnaire survey of professional dermatologists in our hospital,400 clinical data were collected and collated.The latent category analysis method was adopted,and R statistical software was used to explore the internal relationship between observation variables(TCM symptoms)and latent variables(TCM syndromes),so as to explain the correlation degree of external variables with the minimum number of latent categories The analysis steps of potential category model include model parameterization,parameter estimation,model identification,model evaluation,potential classification and result interpretation.Results Taking the minimum BIC/AIC calculated value as reference,it was concluded that the latent variable model was the best model for fitting test when the latent category was 3(AIC=36908.18,BIC=39677.15)According to TCM syndromes(TCM symptoms and signs include tongue and pulse),the conditional probability characteristics of each item under each potential category are interpreted by potential categories.According to posterior probability,the TCM syndromes of 400 patients with psoriasis vulgaris can be grouped into 3 categories,namely,damp-heat syndrome(42.15%),blood-heat syndrome(39.54%),yin deficiency and blood dryness syndrome(18.31%).The number of patients with each syndrome type is 182,168 and 50 respectively.Conclusion Traditional Chinese medicine syndromes of psoriasis vulgaris in Yunnan are damp-heat syndrome,blood-heat syndrome and yin deficiency and blood dryness syndrome,of which damp-heat syndrome is the most prominent,but its main line is still "blood component".In the clinical diagnosis and treatment process,attention should be paid to clearing heat and promoting diuresis through clearing heat and cooling blood,as well as nourishing yin and moistening dryness.
