| Objective To observe the clinical effect of acupuncture combined with the three-step massage for the treatment of knee osteoarthritis and explore its effectiveness to offer an effective clinical treatment of knee osteoarthritis.Methods 78 patients who were diagnosed with knee osteoarthritis in the Hubei University of traditional Chinese medicine affiliated policlinic-Guoyitang from December 2014 to November 2016.The patients were divided into the treatment group and the control group randomly,39 patients in each group.In the treatment group,36 cases were treated,and the rest 3 patients quited due to work assignment and other diseases.In the control group 38 patients completed the treatment with one giving up because of syncope.The treatment group was treated with the acupuncture combined with the three-step massage.Acupuncture points selection:Dubi,Neixiyan,Liangqiu,Xuehai,Heding,Yinlingquan,Yanglingquan,Guanyuan,Sanyinjiao,Xuanzhong.Manipulate the needles by pulling,pushing and twisting until the qi came.The three-step massage method was mainly based on Zhang Tao,the researcher with the China Academy of traditional Chinese medicine,which was acupuncture treatment 20 min and massage therapy 10 min.The control group was treated with conventional acupuncture treatment and three-step massage.Acupuncture points selection:Xiyan,Liangqiu,Yanglingquan,Xuehai,Ashixue,Dazhu.Add different acupoints according to different syndromes such as Yaoyangguan for the coldness and dampness blocking,Geshu for the blood stasis;Ganshu,Shenshu and Qihai for the liver and kidney deficiency.Acupuncture treatment 20 min and massage therapy 10 min.Instruct patients to do the quadriceps exercises at home every day.All treatments practiced three times a week and four weeks a course.The two groups were scored with VAS,JOA and WOMAC after one course was completed and then analyzed the data with the statistical software SPSS19.0.Result 1.Compare patients' VAS,JOA and WOMAC score of gender,age,course of disease between two groups.The difference was not statistically significant(p>0.05).2.Clinical effect: after 1 course of treatment in the control group there were seven patients cured(19.4%),18 treated effectively(50%),nine improved(25%)and two failed(5.6%)and the total effective rate was 94.4%.In the control group there were three patients cured(7.9%),12 treated Effectively(31.6%),14 improved(36.8%)and nine failed(23.7%)and the total effective rate was 76.3%.After been tested by chi square test,the difference was statistically significant(p<0.05)which suggested that the method used in the treatment group worked more effectively than the one in the control group.3.Compare the VAS score of two groups before and after treatments: the treatment group scored 4.27±1.45 before and 1.33±1.01 after.The control group scored 4.45±1.01 before and 1.14±0.72 after.The analyzed results showed that the differences were statistically significant(p<0.05),which indicated that the acupuncture method combined with three step massage and the conventional acupuncture method both can improve the condition of patients with knee osteoarthritis in a certain extent,and there was no statistically significant difference between two groups(p>0.05).4.Compare the JOA score of two groups before and after treatments: the treatment group scored 60.28±5.73 before and 88.06±5.25 after.The control group scored 59.47±8.69 before and 72.37±4.89 after.The difference was statistically remarkable(p<0.05).The pain was alleviated in both groups but the method used in the treatment group worked better than the one in the control group(p < 0.05)which showed that as to the treatment of knee osteoarthritis the acupuncture method combined with three-step massage was more effective than the conventional acupuncture method.5.Compare the W0 MAC score of two groups before and after treatments: the treatment group scored 48.50±3.74 before and 17.41±3.56 after.The control group scored 47.61±4.03 before and 29.59±3.67 after.The analyzed results suggested that the differences were statistically significant(p<0.05)which proved that methods used in two groups both worded in changing the data in a good way and the acupuncture method combined with three-step massage was more effective than the conventional acupuncture method.Conclusion This study indicated that methods used in the treatment group and the control group were both effective in treating knee osteoarthritis,and they also worked effectively in alleviating patients' joint pain,however there was no evidence in showing which one worked better.And obviously the acupuncture method combined with three-step massage had more advantages in improving joint function. |
