Objective: knee osteoarthritis(KOA)is a common degenerative disease of knee joint.There are not effective therapies in clinic at present.With the application of metabolomics technology in traditional Chinese medicine,it is increasingly highlighted that the Chinese medicine overall thought and the characteristics of metabolomics is clearly a high degree of fit.Acupuncture has been very important as an integral part of Traditional Chinese Medicine.Different stages of KOA and different intervention ways,acupuncture for knee osteoarthritis has different curative effects and its mechanisms.In this study,metabolomics technology based on LC-MS was used to analyze the changes of metabolites in serum samples of rats in each group,disease biomarkers and metabolic pathways of KOA were screened.Furthermore,therapeutic effect and mechanism of the staging treatment of acupuncture at distant and near points on knee osteoarthritis in rats were explored,so as to provide theoretical basis for the clinical application of KOA.Methods: 1.Male SD rats were randomly divided into control group and model group.The rats in the model group were injected with collagenase into the right knee joint cavity,and the rats in the control group were given normal saline.After 6 weeks,the serum samples of the rats were collected,they were examined by liquid chromatography-mass spectrometry(LC-MS),and used multivariate statistical methods to find differential metabolites.Screen for differential metabolic pathways with online analysis software.2.Male SD rats were randomly divided into control group,model group,the group of local acupoints and the group of far and near matching acupoints.Acupuncture intervention was given three days after modeling,and acupuncture treatment was given five times a week.The intervention effect of acupuncture treatment was observed by open field test,mechanical pain sensitivity test and thermal pain sensitivity test eight weeks after modeling.The rat right knee joint cartilage were collected,the changes of TNF-?,IL1-?,Leukotrienes,PGE2 contents on rat serum were measured by enzyme-linked immunosorbent assay(ELISA),morphological changes of rat knee joints were observed by HE staining technique.Results: Based on LC-MS,the results showed there were 306 different metabolites in control group compared with the KOA group.Nicotinic acid,carnitine,glycocholic acid,adipic acid,15 keto prostaglandin F1 A,D-glucosamine acid were decreased,arachidonic acid,ethyl 3-hydroxyacetate,LPC and other substances were increased in the model group compared to the control group.A total of 20 differential metabolic pathways were selected,among which four metabolic pathways,including arachidonic acid metabolism and steroids biosynthesis,c AMP signal transduction pathway,and pentose phosphate metabolism pathway,were the main pathways contributing to metabolites in the KOA rats.2.Both local acupoints and far and near matching acupoints groups relieved knee hyperalgesia,reduced the levels of TNF,leukotrienes,IL1-?,PGE2 and attenuated cartilage degenerationca compared with the model group(P?0.05).Compared with the long-term outcome of local acupuncture,the long-term outcome of far and near matching acupoint acupuncture is increased(P?0.05).Conclusions: 1.KOA metabolic changes obviously,metabonomics can screening potentialand related pathways,including steroid hormone biosynthesis,c AMP signaling,and pentose phosphate metabolism.2.Acupuncture has some advantages for the treatment of knee osteoarthritis,which is able to reduce inflammation in the knee joint,protecting the cartilage.There was little difference between local acupoint retrieval and far and near acupoint alignment in the treatment of acute knee osteoarthritis.However,far and near matching acupoints have certain advantages in the treatment of chronic knee osteoarthritis.
