Objective:Based on healthy diet, aimed to compare effects of aerobic training and resistance exercise training on the cardiovascular risk(Framingham Risk Score(FRS)and Ischemic Cerebral Vascular Disease Risk Score)and glycaemic control, lipid profile, blood press, insulin sensitivity, body composition, plasma homocysteine(Hcy)and serum cystatin C(Cys C)after 6 months of exercise among patients with type 2 diabetes.Methods:80 patients with type 2 diabetes were randomly divided into 2 groups:aerobic group(n=40),and resistance group(n=40).Both training must reach medium intensity. And 2 groups must finish 50min each time,3 times per week and adhere to 6 months consecutively. Before training,80 patients were instructed by health care professionals on healthy diet.Results:80 patients were recruited in the study, after 6 months, the data of 65 patients were finally analyzed:aerobic exercise(n=33),and resistance exercise(n=32).Between 2 groups, the baseline was comparable(P>0.05).After exercise,The difference value of waist circumference(WC),muscle mass and Hcy concentrations(6 months later-baseline)between 2 groups were significant difference(P<0.05).Within resistance group, muscle mass,HDL-C,FINS,HbAlc,Hcy,the risk of FRS and coronary heart disease(CHD)were significantly different before and after the 6 months' intervention(P<0.001,P=0.021,P=0.026,P=0.026,P<0.001,P=0.029,P=0.017,respectively).Correspondingly,in aerobic group, there was statistically significant in WC,waist hip ratio(WHR),FPG,HbAlc,HDL-C,TC,serum Cys C,the risk of FRS and CHD(P=0.001,P=0.032,P=0.006,P<0.001,P=0.026,P=0.020,P=0.021,P=0.003,P=0.002,respectively)Conclusions:1.Compared with aerobic exercise, the resistance group in improving the patient's muscle mass and Hcy levels was better. And aerobic group in reducing WC was better than resistance group.2.Between resistance and aerobic exercise groups, there was no statistically significant in the ICVD risk, but there was a tendency to improve in two groups, and resistance exercise was better.3.Within 2 groups, after 6 months, both resistance and aerobic exercise had no effect on 2hPG,insulin resistance and the risk of ICVD.4.Both resistance and aerobic exercise can improve the level of HbAlc,HDL-C and FRS and the risk of CHD.5.Within resistance group, exercise can effectively increase muscle mass, reduce the level of FINS and Hcy.Within aerobic group, exercise can lower WC,WHR,the level of TC and serum Cys C.6.Resistance and aerobic exercise were equally effective in improving HbAlc,HDL-C and CHD risk, and both were worth recommending.Resistance exercise had a significant advantage in increasing muscle mass and reducing the level of FINS and Hcy.For the elderly, maintaining muscle strength, and reducing Hcy concentrations and the risk of atherosclerotic disease and stroke, resistance exercise is recommended.
