Background:Colorectal cancer is one of the world's common tumor,and some patients are found to have acute complete or incomplete intestinal obstruction when seeking for treatment.Emergency surgery is the first choice for the treatment in the past.However,due to the general situation of patients with acute surgery,such as water electrolyte imbalance,poor nutritional status,so as to increase the risk of surgery and postoperative complications.since the use of intestinal obstruction stent could change the emergency surgery to elective surgery,and decrease the operative risk and postoperative complications.Objective:To evaluate the efficacy and safety of intestinal obstruction stent by comparing short-term efficacy of elective surgery after stent insertion and emergency surgery on patients with obstructive colorectal carcinoma.Methods:A retrospective analysis was made on the clinical data of 59 patients with obstructive colorectal carcinoma who received treatment in the Department of Colorectal anus Surgery of the Qilu Hospital of Shandong University from April 2014 to May 2017.According to treatment method,these patients are divided into two groups:emergency operation group(n = 34)and intestinal obstruction stent insertion group(n = 25).Clinical data of patients were collected,and the rate of one-stage resection and anastomosis,operation time,number of lymph node dissection,postoperative anastomotic leakage,postoperative abdominal infection,incision infection,postoperative drainage volume,postoperative exhaust time,hospital stay,total hospital expenditure.Results:There were no significant differences between the two teams in sex,obstruction site,pathological type,tumor stage,age,diabetes,hypertension,heart disease and so on(P>0.05).There were no significant differences between the two groups in the number of lymph nodes dissection,operation time,abdominal infection rate,perioperative mortality,postoperative drainage and postoperative exhaust time(P>0.05)Two groups of patients with I stage resection and anastomosis rate,postoperative anastomotic leakage,wound infection,hospital duration,the total cost of hospitalization was statistically significant(P<0.05).Conclusions:The treatment of obstructive colorectal carcinoma with intestinal obstruction stent:1intestinal obstruction stent can release the obstructive symptoms in about 24h;2?intestinal obstruction stent could change the emergency surgery to elective surgery,and reduce the postoperative complications and mortality;3?To be a palliative treatment method to replace the enterostomy,patients could avoid the anal bag and can have a good quality of life.It is a safe,effective and feasible method for the treatment of colorectal carcinoma which is with an acute intestinal obstruction situation.
