Abstract/Summary:
Objective To investigate the difference of early postoperative inflammatory intestinal obstruction in the prevention and treatment between laparoscopic and open radical resection in patients with colorectal cancer.Methods From September 2000 to January 2006 ,484 cases with colorectal cancer were divided into two groups prospectively and nonrandomly (laparoscopic surgery.LS and open surgery.OS). The incidences of early postoperative intestinal obstruction of two groups were compared.Meanwhile some experiences of the prevention and treatment were concluded .Results 1.In LS group,14 cases (6.22% ,14/225) were converted to open surgery ,because of 7 cases with intraoperative complications, 5 cases with obesity or huge tumors ,1 case with narrow-pelvis and 1 case with retroperitoneal tumor;2. The incidences of early postoperative inflammatory intestinal obstruction(EPIIO) of LS group and OS group were respectively 3.32%(7/211) and 7.72%(20/259),P<0.05(X2=4.166);3.One early postoperative mechanical intestinal obstruction happened in LS group was Trocar hernia. It was cured by operation of hernial ring solution and small intestine reposition.Conclusions 1.The incidences of EPIIO are statistically significant between two groups.Laparoscopic colorectal cancer radical operation can decrease the incidence of EPIIO.2. EPIIO should take concervative treatments primarily.If early postoperative mechanical intestinal obstruction has been diagnosed,such patients should be operated to relieve intestinal obstruction.3. The laparoscopic colorectal cancer radical operation is safe and feasible.
