Objectives Compare two kinds of therapies, namely the auricular-plaster therapy +Chinese herbal fumigation and auricular-plaster therapy + Chinese herbal fumigation +five-tone therapy, on their effects of pain management and relief of concomitant symptom,and, on the basis of which, evaluate the effect of combined use of intervention methods to alleviate postoperative pain, reduce concomitant symptom, improve the quality of postoperative life of patients with mixed hemorrhoid and improve their self-care ability.Methods Chose 150 patients, diagnosed with mixed hemorrhoid and aged from 18 to 60 years, in the third anorectal apartment of Tangshan Hospital of Traditional Chinese Medicine between January 2014 and June 2015. The diagnostic criteria was the Guide of Clinical Diagnosis and Treatment of Hemorrhoids published in September 2006 by Division of Colorectal and Anal Surgery, Branch of Surgery, Chinese Medical Association,Professional Committee of Anorectal Disease, China Association of Chinese Medicine and the Professional Committee of Colorectal and Anal Diseases, Chinese Association of the Integration of Traditional and Western Medicine, and the operation method was external incision and internal ligation. Selected cases were randomly divided into 3 groups, namely the control group(routine nursing care), observation group 1(routine nursing care +auricular-plaster therapy + Chinese herbal fumigation) and observation group 2(routine nursing care + auricular-plaster therapy + Chinese herbal fumigation + five-tone therapy),50 cases in each group. We respectively evaluated patients' anxiety, sleep, self-care ability,life quality, pain and defecation condition based on questionnaires issued on the preoperative day, the day of the surgery(after operation), the 1st, 3rd, 5th and 7th postoperative days. We used spss17.0 for data collection, collation and statistics, and evaluated the effect of combined use of intervention methods to alleviate postoperative pain, reduce concomitant symptom, improve the quality of postoperative life of patients with mixed hemorrhoid and improve their self-care ability.Results 1 Effects of different interventionprogramson pain in patients undergoing hemorrhoids surgery : Comparison between the case groups and the control group immediately after the surgery revealed that the degree of pain in the observation and at the1 st, 3rd, and 7th day after the surgery,the degree of woundpain in the observation group 1and the observation group 2 were both lower than that in the control groupat dressing changen which demonstrated a statistically significant difference(P<0.05); in addition,there was a statistically significant difference in the degree of woundpain between the observation group 1 and the observation group 2 under non-excited state(P<0.05).2.Effects of different interventionprogramson relevantsymptoms in patients undergoing hemorrhoids surgery:(1) Urinary functions: Compared with the control group,first micturition time, urinary flow rate, micturition status andthe symptom score of pain in the lower abdomen were all superior, and the number of cases of urethral catheterizationwas lower in the observation group 1 and the observation group 2(P<0.05);no statistical significance was found in above indicators between the observation group 1 and the observation group 2(P>0.05);(2) Defecation function: Comparison of Wexner constipationscore among the 3 groups demonstrateda lower score in the observation group 1 and the observation group 2 than the control group(P<0.05),andcomparison between the observation group 1 and the observation group 2showed a lower constipation score in the observation group 2 than the observation group 1(P<0.05);(3) Bleeding during defecation:Comparison ofbleeding during defecationamong the 3 groups revealed milderbleedingin the observation group 1 and the observation group 2 than the control group(P<0.05);as compared with the observation group 1, bleeding during defecation was milder in the observation group 2(P<0.05);(4) Edema of anal edge: As for edema of anal edge,the observation group 1 and the observation group 2both presented milder edema when compared with the control group(P<0.05), while no statistical significance was observed between the observation group 1 and the observation group 2(P>0.05);(5) Self-care ability:Comparison in self-care abilityafter surgery among the 3groups indicated a superiority in both observation group 1 and observation group 2 than the control group(P<0.05),while no statistical significance was detected between the observation group 1 and the observation group 2(P>0.05);(6) Anxiety state:As compared with the control group, both observation group 1 and observation group 2 showed milderanxiety(P<0.05), and comparison between the observation group 1 and the observation group 2 revealed milderanxietyin the observation group 2(P<0.05);(7) Sleep quality:Comparison of sleepconditions after surgery among the 3 groups showed a superiority in both observation group 1 and observation group 2than the control group(P<0.05),as compared with the observation group 1, sleepconditions in the observation group 2were superior(P<0.05);(8) Quality of life:As forquality of lifeafter surgery, both observation group 1 and observation group 2 demonstrated a superiority as compared with the control group(P< 0.05), and comparison between the observation group 1 and the observation group 2 showed higher quality of lifein the observation group 2(P<0.05).Conclusion 1 Combined application ofauricular point stickingand Chinese herb fumigationcan alleviatepostoperative wound pain, improveurinary functions, reduce the rate of urethral catheterization, ameliorateconstipation status,decrease bleeding during defecation, easeedema of anal edge, enhance postoperative self-care ability, relieveanxiety,andimprove sleep quality and quality of life; 2 Combined application offive-tone therapy withauricular point stickingand Chinese herb fumigationcan alleviatewound pain under non-excited state, improve constipation status andreduce bleeding during defecation, as well ashassuperior effects on anxiety relief and improvement ofsleep and quality of life.
